when i took medical in st.lukes nurse said i still have 2 vacination need to get but here in u.s. any one knows this vaccination thingy? where and how i can get this shots?

Nov. 2009 -- GC for 2 yrs received

Jan. 2010 -- DL issued

May. 2011 --- graduated for my bachelor degree

Sept. 1, 2011 --- Driver License Renewed ( till 2017)

NO removal condition

Citizenship

Oct. 2011-------- Filed N400

Jan. 2012 ------ Biometrics and Interviewed at same time

Jan. 17 2012 ---- Sworn In

If you know what vaccinations you need you can get them from any health clinic, doctor, etc.

Naturalization

3/23/14 - N400 package sent to Phoenix

3/27/14 - N400 package delivered

4/3/14 - NOA1 receipt date

4/4/14 - check cashed

04/29/14 - biometrics date

07/01/14 - interview date

xx/xx/xx - Oath Ceremony

As we understand it you have to go to a Civil Surgeon in your area to get the rest of your shots.

There is a site in USCIS with all the Civil Surgeon in your area.

Rosie filled out the form I-693 and took that and her shot paper she got from St Luke to the Civil Surgeon.

You don't need a Physical Exam but they will look over the shots.

Rosie need the HPV 3 shots .. she had one so they gave her number 2.

and one other shot I don't remember.

THE DOCTOR UPDATES THE I-693 AND ADDS THE SHOT HE GAVE.

HE GIVES YOU AN ENVELOPE (DON'T OPEN) TO SEND WITH YOU AOS FORMS.

HE GIVE YOU A NEW COPY OF WHAT HE HAS IN THE ENVELOPE FOR YOUR RECORDS (LAUGHING so you dont have to peek in the envelope anyway).

NOTE: Rosies AOS I-485 has take 6 months so we took her back to the CIVIL SURGEON and got her last HPV shot. and we had him add that to our copy of her shots .. NOW when we go for our Green Card INTERVIEW we will show them that she has all of her shots.

I think it's that you have to go to a civil surgeon for an updated medical form if you need one for AOS...

Vaccinations can be given by any Doctor, then records of the vaccinations can be provided for the updated medical form.

yes... there's a record from slec (vaccination sheet) that will tell the doctor what type of shots should be given...

MythaG: just present that sheet from slec to any qualified doctors you wish to get a shot from, and or i had my shot from a health clinic/center in our area and it didnt cost a thing (as i remember)... since your a CR1 i believe there's no pressure as to when you can do it since you're done w/ the AOS... (IMHO)

My wife is in the same situation. She needed HPV #2 and Varecella (chicken pox). We went to the local county health dept for the shots. Local doctors usually don't stock the shots you will need (ours didn't). Our USCIS doc gladly accepted the receipt from the health dept as proof and filled out our 693.

You should have a vaccination report from them. Ours listed all the vaccinations and whether she had them or whether they were inappropriate for one reason or another. I'd look that over first and see what ones they are talking about.

Notice that .MythaG- came on a CR/1 immigrant visa and not a K non-immigrant visa. .MythaG- has no need to adjust status. .MythaG- could send the DS-3025 to Dr. L E Arnold in Austin to find out what vaccinations are needed if she does not know.
