:( K1 Visa needs vaccine records right? My vaccines like BCG,DPT,Polio, Hep,Measles,etc. I dont have my records of 'em coz those files 10 years from present date they told me they were dispatched already,so I cant get a record nymore...and my copy of those vaccines, I cant seem to find them... But I really did got a shot each of those vaccines... :bonk: Can I just make a certification or a vaccine record myself tellin that I did got those vaccines? coz My hep vaccine was given by my pediatrician...but the others were from another hospital...and I talked to my pediatrician and agrees with me that on behalf of my other vaccines she will sign,considering that we traced the dates of the vaccines on when did the health care providers gave the vaccines to me, my basis is (my mom coz so far, she can still remember when did those vaccines given coz it was like days and weeks after my birth date).. Can someone please give me a format on makin this vaccine stuff? :help:

Request a titre test for the specific vaccinations and immunitys will show up if you had the shots, if no immunitys show up then you will know you need to retake the shot. Bring the titre test to your k-1 medical and they will judge which ones you need to get. You cant simply write down that you got them they wont accept that.

Any pathology lab would be able to give you a titre test for the vaccines that are in your blood. It's pretty common for people to not have or have lost the vaccination records. The titre test provides results that confirm the said vaccines are present / or not present in your system.

Someone just pm-ed me and told me that, I wont be needing to repeat the test... I wont be needing that test :unsure: and I will just go to St. Lukes and pay $213 and they will give me what I need ... w-what was that? :blink:

All shots are age appropriate, you be given only what is required at St. Lukes.

there are panel physicians who will interview you for the vaccinations. it's okay if you don't have your immunization records, they're just gonna give you the vaccines that are appropriate for your age. when i had my medicals on oct26th and 27th, they gave me anti-cervical vaccine, measles/chicken pox (coz they've asked me if i had chicken pox i said no), tetanus toxoid, i forgot the other one. i received 2 shots each both arms and the most painful shot was the anti-cervical vaccine! so don't worry ok? you won't be needing the immunization record. :yes:

Improved USCIS Form G-325A (Biographic Information)

Form field input font changed to allow entry of dates in the specified format and to provide more space for addresses and employment history. This is the 6/12/09 version of the form; the current version is 8/8/11, but previous versions are accepted per the USCIS forms page.
