hi vj members im here in states in a K1 or Fiancee Visa, were gonna file my AOS this coming 1st week of december and i am wondering about what my lawyer said that while waiting for december we should have to set an appointment to a doctor to check my vaccination records. Is this really required in filling the AOS? i have a 4 shot of vaccinations already in my country b4 i came here and i have my complete medical records. i am just thinking this will be additional expenses again for us for the vaccination if ever so b4 we will proceed to it i just wanna make sure that my fellow AOS fillers were doing the same procedure also as consulting to a doctor for the vaccination record.

Hi buddy,

This is true that you need to take appointment with local surgeon for vaccination supplement even if you had medical examination in your country earlier. This is not complete medical exam but supplement vaccination which is recently updated. Please let me know that what are the documents you got from the hospital/doctor who took you medical exam in your country.

Hopefully you have the DS 3025 vaccination records that was given to you at St. Lukes.

Call Dr. Arnold [uSCIS Designated civil surgeon] and he'll transfer the vac records to the DS 693 for $30. LOTS of people here on VJ use him. He did it all by mail. Takes about 10 days.

Yes you will need to have a USCIS doctor review your shot records. Even if you do not send in the I-693 and do not get an RFE, you will still need the signed form during you AOS interview. My wife also had 4 shots in St Lukes and required 2 more here (HPV and Varacella aka chicken pox). remember you cannot just go to any doctor to have your records reviewed, he or she must be USCIS approved. I used Dr. L.E. Arnold in Austin Texas (we are in florida). Call them and they will have you send them a copy of the vaccination record from St Lukes. They will call if you need any more. The total charge is 30 USD, a lot less and easier than traveling many miles just to get a shot record looked at. My nearest USCIS doctor is in Jacksonville Fl and wants 3 weeks for appointment and 175$. No thanks! Dr. Arnolds office will tell you what shots you need then go to your own doctor or county health dept for the shots then send the receipt to Dr. arnolds office who will sign your new I-693 for your AOS package. Here is their info. Call them and they will explain what to send to them. Hope this helps
