hi to everyone!!!!

me and my husband already print all the forms to be needed in filling the change of status.. we got married last Sept 06 in Las vegas.. we just want to know what surname should i used in the form? is it the surname when i was still single? or my husband surname???? and pls tell us how many picture do i need??? pls help us... :help:

I first met my fiance on November of 2004 here in the Philippines.. A friend of mine introduced me to him at a birthday gathering. After that meeting he kept in touch with me through out the duration of his vacation either by phone or by visiting me at my place. Before he left he promised me that he will call me everyday and would come back to see me again the following year..

As he promised, we kept in contact and called me almost everyday. From this, we became close and fell in love. He came back a year later as he promised on November of 2005. We took a trip to HongKong to visit the then newly opened Hongkong Disneyland. Its d best vacation i ever had... After spending time with each other we had realized that we really enjoy each other's company and we've discussed about marriage. He visited a few more times and then finally on his last visit on September of 2007 and almost three years of knowing each other he proposed to marry me and i happily said yes!!!! I cant described my happiness that day.. I want to cry because i really never expect he will proposed to me...

Surname:

You can use either your maiden/single name or new married (husband's) name on the I-485 (and related) forms. It is up to you; which ever you choose is what your PR/Green Card will say. Your marriage certificate should show the married name you chose (most states).

Photos:

The only photos you need to send are 2 passport style photos.

“...Isn't it splendid to think of all the things there are to find out about? It just makes me feel glad to be alive--it's such an interesting world. It wouldn't be half so interesting if we knew all about everything, would it? There'd be no scope for imagination then, would there?”

Hi,

File number is the same as your A-Number, written on your visa (the number beginning in A).

As for the I-693, the civil surgeon should have the form. Or you can fill in part 1 yourself before you go, but don't sign it, and bring it along with you. I'm guessing you had a medical exam done for the K-1 so just bring your vaccination information with you and the doctor will fill in the vaccination part of the form and Part 5. The rest of the form is not necessary. They will give it back to you in a sealed envelope. Make sure the civil surgeon you go to is one approved by USCIS. It cost me $25 to get a civil surgeon to do this for me (in Kansas).

For I-765, put in your maiden name in the space for 'other names used' and write 'maiden name' in parenthesis.

'Which office' applies to you only if you applied for Employment Authorization before, so just put N/A if this is your first time.

'Manner of last entry' and ' current immigration status' are both just 'K1'.
