no one wants to just fill out the form....they all want to give blood tests and re-do the physical...well it has to get done right???

i was talking to someone the other day and they said coming off of a k-1 it doesn't need to be done....i am ssoo confused...

Sorry, I can't help you. We didn't have to do this when Tony came here. All I did was make a copy of his vaccinations that he brought from the medical and that was fine. I freaked out because I thought everyone had to get the supplement, but we didn't and we got the green card w/ no problems. B)

Does that form need to be done in order to send it with the packet? I just made a copy of his vaccination record.

I smell a RFE coming my way...

That's all we did too and NO RFE  But, that was 1 1/2 years ago!

Still wanna see Barbados, followed by TnT. After that maybe we'll check Aruba.

I loved Barbados, but it was really expensive! We spent over $5200 in one week :o You can do it a little bit cheaper, but we wanted to stay somewhere nice (Hilton). I'll try to send you some pics.

Have any of you guys had a problem with with your spouse not wanting to go back to Jamaica for the next four years ? My husband stated last night that he would not be traveling back to the Yard untill 2010.

Why doesn't he want to go back to JA?? I think most of us have had the opposite problem! They want to go back as often and as soon as possible. I know Tony misses his family and friends. I can't imagine him staying away from them for 4 years :o That's a long time.

[/quote ]

As he staes you need to go back with big pockets I guess he's trying to prove something.

for everyone going to need a civil surgeon... i just got this pm today,,,i'm going to check it out to make sure it is the real deal...the girl who sent this to me had her case transferd to cali...so it worked for her...

Jamie found a Doctor who was doing this too -- and it worked for a lot of people -- until they went out of business or something. Can't hurt -- go for it!! The price is right. Try to make sure they send you a copy of what is in the sealed envelope -- just so you know. Yipee!!

Would it be totally wrong to go without him  .

Girl, dont argue. Not worth it. Go to another island, there are sooooo many to choose from. Besides that it would be nice to show him other cultures since he has lived in Jamaica most of his life. You can go to the US virgin islands, Mexico, Belize......Or shoot if he really wants a change take him skiing!!! Ha.

I guess I was just getting nervous that I did something wrong. Where does the DS3025 come from? Bert did not get any paperwork from the doctor. He has to get his 3rd hep shot and a tetnus shot.

WE ARE DONE!!!

HERE IS THE INFO!!!!!!!!!

after doing some research...the ds-3025 is the worksheet from there medical....you are supposed to ask the DR for a copy...i didn't know this and i am sure many other didn't know that as well...so for everyone who's SO is going for the medical remind them to ask the DR for a copy of the worksheet you will need it to make you life easier and your pay smaller!!!!

Ok so for those of us who do not have this some dr's may just take the vac. paper and but it on the i-639a OR may do a whole exam....i guess it depends on who you call....OR you can call the place they went to in JA and ask them to fax you a copy of the worksheet

i have also read that some people just send in the ds-3025 with the packet of info and some have gotten rfe's some have not....

i hope this helps clear up some of the confusion!!!

ok here is a good question....what is all mike has is his vac. paper saying all the shots he got....but he doesn't have a ds-3025 form....what do we need to do...who would i call???

Verton received a card that listed his vaccination on it. It was like the one you received when you where in school. It had a print out of the Dr's name address and dates of vaccination.
