hello all :star:

starting to assemble our AOS packet and this DS 3025 (or sometimes called DS 2053) still confuses me. :bonk: I do not have this DS 3025 as far as checking on any forms left with me after passing the K1 visa except my original vaccination record from where i did my medical.

It says on the I-693 instruction to provide this and "your" DS 3025 to the civil surgeon. Further says, if u do NOT have a copy, you need to provide sufficient evidence of vaccination history....

so, is my vaccine administration record the so called DS 3025?? :help: (id like to think it is except that i cant find anywhere it written DS 3025 :huh: )

hmm.. ive been reading more about this DS 3025 and thankgoodness its not just me :innocent: ... i wish i had requested copy of my medical if i have known it before.... nway, it seems there is a medical form where its written "DS 3025" which obviously i didnt have with me right now. :crying:

No, this is not the DS3025. The DS 3025 is given to you at the medical exam, not the consulate, and you keep it for use with the AOS application.

My advice is contact this civil surgeon in Texas, Dr. Arnold, he will fill out an I-693 based on your vaccine evidence for $30. His name and address are around the site her. Do a search. You can submit the I-693 OR the DS 3025.

FYI The DS 3025 should be given to you at the medical exam, bring your vaccination records with you to the medical exam, do not leave until they have completed the DS 3025

No, this is not the DS3025. The DS 3025 is given to you at the medical exam, not the consulate, and you keep it for use with the AOS application.

My advice is contact this civil surgeon in Texas, Dr. Arnold, he will fill out an I-693 based on your vaccine evidence for $30. His name and address are around the site her. Do a search. You can submit the I-693 OR the DS 3025.

Hi,

For the AOS package, you have to submit Form I-693. But because you already had a medical back home, you only need to fill in Part 1 (but don't sign it yet), and get a civil surgeon to fill out the vaccination section of Part 2, and Part 5. The civil surgeon must be a USCIS-approved civil surgeon. You can find an approved surgeon on the USCIS website on its civil surgeon locator. You just type in your zip code and it will give you details of the approved civil surgeon nearest to where you live.

When you go to see the civil surgeon, you need to bring them the vaccination record you got from the medical you did before coming to the US. The civil surgeon will check to see if you have all the required vaccinations, and if so, will fill in the vaccination part of Part 2, and Part 5. They will then give you the form back in a sealed envelope. I paid $25 for this in Kansas.

The vaccination record you got from your medical exam does not need to be sent with the AOS. Just send the I-693 in the sealed envelope.

wow $25 is a good price.. yup..i printed some civil surgeons in our area from USCIS website and we should check one or two this week and if all fails...will contact dr arnold.

btw, with the highlighted above- its more clearer for me that yes i have my original vaccination record from my medical back home , however, i still wonder if this is the so called DS 3025 (or also referred to as). i guess am good to go with my vaccination record- hope we'd find a doc that does understand K1 medical procedure like u did.
