I'm sure others further into the process than me will have more to add but here's my answers to start you off:

Eligibility for I-765 - I put C9

The I-693 you can either download or the civil surgeon that does your medical should have it too. It's part of the I-485 and while theres no charge for submitting the form- you do have to pay for the medical.

There's a list of USCIS approved civil surgeons on the Uscis.gov website on the left hand side. My medical cost $125 plus I was missing 2 vaccinations. My civil surgeon didn't do the vaccinations so I went to another doctors to get them, I don't have the bill yet but they quoted me $60 - $70 per vaccination.

The I-864 you don't necessarily need a tax transcript if you have a full copy of your tax return plus W2s and didn't telefile... But if you don't have all your tax return then yes you do need the tax transcript.

Also while it seems like information overload at the beginning, you'll be surprised how much you learn in a month or so. Just reading these forums and reading the form instructions in your spare time, definitely helps. It will all make sense soon!

Not quite. When you file the I-130 and the I-485, there is no fee for the I-131, I-765 or i-864. You are correct that the 130/485 combined are 340+1010. You will need a physical, and there will be costs involved in that.

JERIII: thanx..heh yea tell me about it..i had to pay 120 for fysical...and 75 for all the shots.

thing is though..after all the 4 shots (hep a+b, mmr, isip and something else)

the nurse asked me if I wanted a FLUE shot.. i said no cause it costed like 20 bux...

now..is that required ?

second Q:

my wife´s sayin we should send in i-130 FIRST..get something back from them..THEEEN send aaaaaaaaaaaall the forms etc ...

i believe we should send in aaaaaaaall and the checks etc at the same time...what´s the best thing to do ?

Hey! I'm in your boat mate... also on F1 married to a US citizen and doing adjustment of status now, we just sent in all the papers without the help of a lawyer a few days ago. Don't send in I-130 seperately and first, send it All together, will save a lot of time... we sent in about 4 sets of papers all in one envelope, fedex'd it to the address stated on the I-130 instruction sheet.

I included a coverletter infront of each stack listing the contents and paper clipped each stack seperately. (If you want copies of these coverletters I will gladly email them to you, although they are not necessary)

Yes, your wife has to be your "sponsor" for now I guess, if she falls below the poverty line then you have to find another sponsor.. for a household of two I think it's around $13 000 per year or something, not hard to cover if she has a normal job/salary.

"Influenza" or the ordinary flu shot is definitely required as part of your vaccinations, I got one at CVS minute clinic as everywhere else they were out. Make sure you get all your vaccinations together, take it to your doc and make sure you get a copy of your lab report/physical from the doc and he has to give you a SEALED ENVELOPE to include with your I-485, don't open it! haha

Shani682, are you sure that the flu shot is required for all applicants? I *thought* I did pretty thorough research about what vaccinations I needed and didn't need, and what I read was that the flu vaccination was only required for people over a certain age (maybe something like 65) or high-risk people, e.g. people who'd be more susceptible to getting flu because they already have certain diseases.

My doctor recommended that I get the flu shot because I have asthma but when I went to get the vaccinations done (in early August), I couldn't find anywhere to give me the shot (it was still too early for places to be stocking it, I was told). I told my doctor this, and he said that it wouldn't really matter seeing as my asthma is really mild.

As regards the cost of the medical, mine was $170 and I paid only $10 for all four vaccinations I needed (if you search you may be able to find places that do really low-cost shots for low-income populations).

I just looked at the list of required vaccines (see attachment), and flu is only required if you're 50 or older. If you're younger than that, you'd only need it if you were in a high-risk group.

Before I went for my medical, I made a point of looking over what the vaccination guide said, and I was glad I did, as the doctor told me to get that stupid HPV vaccine, which I was actually too old for. I don't really think my doctor really knew what he was doing, to be honest. I think he offered immigration medicals to make a quick buck, as I was in and out of there in two milliseconds. He had also clearly been an army doctor, as I've never met a doctor with such a terrible bedside manner. He practically manhandled me!

I'm also worried about my TB test, as he never made me get one, as I had already done one when I arrived in the country in 2004. However, when he was filling out the form later, he said "Oh, maybe we should have done that". I'm worried that will cause a problem although hopefully it won't.

you could definetely be right that it's not required,maybe each doc does it differently, but when I went to the doc I basically had NO vaccination record (evidently my parents didn't believe in it, however I've never had any disease/illness, Ever...) they did a TB test (for sure required!) which they checked 2 days later and the required physical and lab work (HIV & Syphillis tests)($179).

Then, the doc wrote out a prescription list of vaccinations that I needed and influenza was on the list - they referred me to the Dept. of health to get the vac shots as it was much cheaper (still $300!), so I got 6 different shots for various things including polio, and I had to go to CVS to get a flu shot as everywhere else was out of it - and sure, everyone else getting the flu shot was way older and I did read it was recommended for 65 years and older but the nurse practitioner giving me the shot knew that I had to get it for immigration purposes and did it...

A note about the actual form, I filled out the first part and printed it and that's the one she used and sealed in the envelope, I'm glad because atleast that way I was sure the form was current, they had a pad of forms that had a whole page different from the one I printed out! and I've heard of an entire I-485 package being returned when the doc used an out of date form!

Yes, I think it's a bit iffy that my doctor didn't give me a TB test. I'm going to be extremely pissed off if I get an RFE because of that. He'll wish he had never met me once I'm finished with him. The medical envelope had already been sealed by the time I started to worry about it, so I decided just to risk it. I'm hoping that it won't matter seeing as I come from a place which doesn't really have TB anymore.

As regards your nurse saying you needed the flu shot, I'm always suspicious when it comes to immigration stuff because I think a lot of people really don't know for sure what you need. Nobody will care about your immigration needs than yourself!

Yes, I made a point of taking along my own form, again because I didn't want to end up with some out-of-date one at the doctor's.

Instructions for I-693, read page 7, it lists influenza, but you're right! I'm not over 65 years old, lol... I'm sure I got it cause I had NO record, oh well...

About the TB test, Oh boy, I think TB is a "communicable disease of public health significance"??

Q. Why am I, as an applicant for adjustment of status, screened for TB?

A. When passing immigration laws, Congress wants to ensure that immigrants with significant health conditions do not endanger the health of the United States public. It put laws in place (section 212(a)(1)(A) of the Immigration and Nationality Act) to guard against certain significant health conditions being spread or introduced to the US.

Therefore, every immigrant, including an adjustment of status applicant, has to be screened for health-related grounds of inadmissibility, such as communicable diseases of public health significance.

Adjustment of status applicants are screened by going to a civil surgeon (a physician that is designated by USCIS) and having a medical examination which includes a screening for communicable diseases of public health significance (such as TB) and other physical or mental conditions. The civil surgeon also assesses vaccination status and administers required vaccines. If the civil surgeon encounters an individual with a significant health-related condition, the civil surgeon will be able, for most conditions, to prescribe the necessary treatment so that the inadmissibility no longer exists and the danger to the public is eliminated or diminished.

Q. Are all adjustment of status applicants screened for TB?

A. Under the May 2008 TB Component of the Technical Instructions for the Medical Examination of Aliens in the United States, and its updates, all applicants for adjustment of status are screened for TB, unless they are too young.

Section 212(a) of the Immigration and Nationality Act reads:

(a) Classes of Aliens Ineligible for Visas or Admission.-Except as otherwise provided in this Act, aliens who are inadmissible under the following paragraphs are ineligible to receive visas and ineligible to be admitted to the United States:

(1) Health-related grounds.-

(A) In general.-Any alien-

(i) who is determined (in accordance with regulations prescribed by the Secretary of Health and Human Services) to have a communicable disease of public health significance, which shall include infection with the etiologic agent for acquired immune deficiency syndrome,

I had the Tb test, but my doctor said if you'd had the test in the last 5 years you didn't need it. I'd last had it in 2003 when I went to school in NY... 6 years ago - so i had to have it again. I'm 27 and my doctor marked age inappropriate to all except MMR, tetanus and chicken pox... The nurse then asked me on my way out if I was going to get gardasil and flu shots. I showed her the form where the doctor had marked inappropriate. I guess it doesn't harm if you've had them but the doctor also can mark inappropriate if he wishes...

I had the Tb test, but my doctor said if you'd had the test in the last 5 years you didn't need it. I'd last had it in 2003 when I went to school in NY... 6 years ago - so i had to have it again.

Well, I got the TB test on August 14th 2004, and my medical form was signed August 11th 2009, so looks like I just managed to get away with it! Hopefully I won't get an RFE for that.

Hi, just have couple of fast LAST questions, last ones we´re finally done with everything thanx to u guys =).

im sending in ALL FORMS at the same time in one package,

now, I put an original marriage certificate along with the form I-130.

The i-485 though, not sure the checklist says to have ANOTHER marriage certificate

with this form. Is that so ? I mean they will see the original marriage certificate

in the SAME package but with the i-130 folder.

2. Why are there so many copies of G-325a forms? according to the checklist, i have

to have eight g325a and another eight g325a for my wife (thats 14 papers looking

exactly like the last 2)

3. In the checklist it doesn't say anything about sending in:

* I-20

* Driver-license copies

Should I send these anyway?

4. Should I include a CD-ROM with around 100 pictures of us in the package? :/
