Just to let people know, since this post is from 2009, the price for Dr. Smith has changed to $250 (yikes!) and the office is incompetent and horrible. Just want people to have accurate information - knowing what we know now, we NEVER would have used this doctor!! They missed half the boxes on the forms, told us twice they would call us back with information about re-filling out the forms (never did), then had to do a whole bunch of other stuff when my husband finally just showed up in person just to get it all done with. Bad stuff people!!

Update on this clinic:

They charged $100 to transcribe to the I-693 form (I was missing varicella vaccine so they just had to add VH since I already had chickenpox). Initially the assistant wanted to do bloodwork for syphilis, despite me telling her that the K1 visa application in the Philippines already required full physical checkup (and I am within 1 year of that checkup). Obviously they will not know the procedures for all visa types in every US consulate in the world, so you have to be clear on what you want when you go to their clinic else they will do everything.

I would rate my experience 6/10. If I hadn't done my research, they could've talked me into doing another full physical checkup (not because they just want the money, but due to lack of information). Also, I had the weird feeling the assistant was just barking out numbers arbitrarily, like the cost to transcribe would vary based on her mood on that day. JUST MY PERSONAL OPINION.
