Hi, I am starting to fill out the forms I'll need to submit for AOS.

I-130 (petitioner prepares)- for the foreign spouse do you enter marriage last name or maiden name?

- Address- do I put address in Canada or now in the United States?

C. Information about your alien relative (continued) 17. List husband/wife and all children of your relative- what name do I put

there?

21. If filing for your husband/wife, give last address at which you lived together. (Include street, city, province, if any, country):

It asks TO and from DATE.

I'm just really confused if I have to but marriage last name on these forms or maiden name??

Does the I-130 & I-864 get filled out by the petitioner and the I-485, I-131, I-765 get filled out by the foreign spouse??

Hi, I am starting to fill out the forms I'll need to submit for AOS.

If you are taking your husband's last name then put your married name. Your marriage certificate will be proof of your name change even if you haven't changed it with everything else by the interview.

- Address- do I put address in Canada or now in the United States?

Put whatever address you are currently living at. If you are applying AOS that means you must be in the US, so you would put your US address.

C. Information about your alien relative (continued) 17. List husband/wife and all children of your relative- what name do I put

there?

You put your name (I'm assuming your the intending immigrant? If you are the USC though then you'd put your husband's name. And list any children's names.

21. If filing for your husband/wife, give last address at which you lived together. (Include street, city, province, if any, country):

It asks TO and from DATE.

So what is your question?

I'm just really confused if I have to but marriage last name on these forms or maiden name??

See my answer above

Does the I-130 & I-864 get filled out by the petitioner and the I-485, I-131, I-765 get filled out by the foreign spouse??

Yes the I-130 & I-864 are filled out by the USC and the other forms you mentioned are filled out by the intending immigrant.

So what is your question? Would I put Sept. 30, 2009- present since I'm still living with him

Yes that sounds right
