Need to fill out I-693 or not? (K3)

Here is our situation: Completed medical last month, received copy of DS-2035 which states that Applicant is eligible for blanket waiver.

So now we see this on USCIS website (updated Sept 02, 2009):

Even if a new medical examination is not required, you still must show proof that you have complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original, overseas medical examination report, you will have to have the vaccination report completed by a designated civil surgeon.

But then in the I-693 instructions (expired August 31, 2009), it says:

You will, however, be required to complete Part 1, Information about you, and submit the vaccination section of Part 2 with your adjustment of status application. A designated civil surgeon must complete the vaccination section and Part 5, the Civil Surgeon's Certification.

Which is I-693 Civil Surgeon's Certification of Vaccination Requirements being met.

Your interview took 67 days from your I-129F NOA1 date.

AOS was approved in 2 months and 8 days without interview.

ROC was approved in 3 months and 2 days without interview.
