hi, we just got married yesterday and i was wondering how the name change is gonna be for me since im filipina. Will i fill out something for it since i want my name to be my first name middle name(maiden surname) then my new family name (husband's family name)

First of, congrats on the wedding!

You can follow the Philippine way of changing single to married name... That way, you won't have a hard time renewing your passport and using the married name at the same time. Just make sure that from now on, all your identifications should be the same name i.e. driver's license, GC, SS card, etc.

I didn't do that way, instead I used my mother's maiden name (which was my middle name when I was single) as my middle name instead of using the maiden name (father's last name). Now that I need to renew my passport, I will have to fill out one and the same form.

Here's how I did it:

Teresa (first name) Reyes (mother's maiden name) Santos (father's last name) - when I was single

Teresa (first name) Reyes (mother's maiden name) Smith (husband's last name) - my now married name and all my IDs are using this name.

In the Philippines, it should be:

Teresa Santos Smith - when you got married. This is also the name that will appear on your passport when you renew it.

It really depends on what you want... like I said, just make sure that all your government-issued IDs bear the same name. :thumbs:

use your husband's last name in all the forms you fill in. in my case my passport was in my previous marriage yet. but i used my maiden name as my middle name. as long as you have your BC and MC supporting your real name and your husband's name, i think there's nothing to be confused there. again use your husband's last name especially in applying the social security number.
