Hi,

I am in the process of completing the adjustment of status paperwork for my husband. For the biographical form (G325-A) do I have to fill out just one form for him and make four copies? Or do I fill out one for myself too (like I did for the K1 visa application).

Also, my husband can't find his imminzation record. He has health insurance here, can he get a new medical exam here in the U.S. and have the doctor fill out the I-693 Medical Examination form? He said his immunation information was in the brown manila envelope he gave to customs when he entered in the United States.

Any information you could give would be very helpful.

the four G325A's are all for your husband (or whomever is immigrating). If its been over a year since the exam for the k1 visa then you must get the medical form filled out by your Dr. here in the US. if it has not been over a year then you dont have to do a whole new exam and you can submit the I693A (i think its A, for affidavit. look at the FAQ to be sure)

i had been over a year for us so we just went to any ol' doctor (who happened to have studied in Sao Paulo and spoke fluent Portugues, yay!) and had him fill out our I693

hope that helps.
