U.K vaccinations done during medical...need to be transferred?

my husband got his vaccinations done in the UK while he was getting his medical exam done. They gave him the immunization record with the signature of the physician. It says on the document, U.S. Department of State for use with DS-2053. Now, Does he still need to get and I-963 (?) filled out by a civil surgeon here in the U.S? I called 2 officies and they are just not understanding it. They are saying about $300. Should I just send the vaccination record he has from U.K in with my I-485 AOS without the I-963?

I am a little confused as to what this vaccine sheet that you have is. Is it the DS-2053??? All you need is the DS-2053 form and you take that to a civil surgeon in your area and they will transfer it onto a 693A supplement. Thats what we did.. it was very easy and only cost $25. Since your husband has already had his medical done it should not cost $300. Just call a civil surgeon and tell them that you nedd a vaccine supplement filled out not the whole exam and you should be fine.
