The embassy in Kiev took my wife and her daughters medical exam documents and sealed them with their package that is handed over upon arrival in the US to immigration. We do not have a copy of their medical exams. I have read that we need to submit a I-693A vaccination supplement form, however we do not have the original medical information to translate over...will I have to have another exam done here in the US? Can they get their vaccinations at the health department and have a civil surgeon complete the I-693A supplement. If someone can give me a step by step solution on how to handle my situation I would be most appreciative for your help.

Did your wife and her daughter receive all the required vaccinations, though, and all that's missing is the documentation?

Then it might be a good idea for them to get a titer test (blood test for antibodies) which would prove that they do not need to be vaccinated again. With the results of that test, you would then go to one of the USCIS doctors (you'll find the list on uscis.gov) and have him fill out the I-693a. You might want to call around for the best price, since the cost varies greatly (between 10 and 120 $ or so).

Alternatively, they could get vaccinated again, and then you would take the proof of the vaccination to the USCIS doctor to get the I-693a filled out.

Personally, I don't believe in being needlessly vaccinated therefore - assuming their vaccinations are all up to date - I would go for the blood test :)

My husband's doctor in his hometown gave us a copy of his vaccination record. We took it to a civil surgeon, and since he had his medical in March '06 all the civil surgeon had to do is fill out the I-693A Vaccination Supplement. No further medical was needed. His medical was also sealed and was not given to him. Civil surgeons that are approved for the USCIS should know about that.

However...some civil surgeons can require that you have another medical done REGARDLESS of having it done within a year of your approval. We ran across this yesterday and it rather ticked me off. It is NOT a requirement of the gov't...it was HER personal requirement and if I had not called around, I would not have found this out. This particular CS wanted $300 to do another medical as well as the I-693A. The one that we eventually found (and went to) charged us only $50 for filling out the form, and for the tetanus shot he needed.

Pays to call and check around, eh?
