I was looking through the "guides" section of VJ and I am getting our I-485 package together. Does anyone know if we have to include the I-693 medical exam form with our I-485 package? In the last part of this guide it say the following:

"Once the Civil Surgeon has has verified that all required vaccinations have been performed, they will complete form I-693 Part 2 (the vaccination section) and Part 5 (the Civil Surgeon's Certification). They will then seal the form in an envelope and return it to you"

Does a Civil Surgeon have to sign papers that you include in your I-485 package at this time or is this for later?

Its really for now. If you don't include it you might you get an RFE which could slow down AP EAD etc. However if you've had all of your vaccinations sometimes you can get away with just including your vaccination from your medical. And yes the civil surgeon must sign it.

If your visa medical was less than one year ago you do not need to re-submit a full medical..... you only need provide any vaccination information

Thanks! I do have her record of some shots done with a "panel Physician's" Signature while she was still in Cambodia. The only problem is that the shots are incomplete(just enough for her to pass immigration and POE). So what you are telling me is that we have to find a civil surgeon and get the rest of her shots and have his signature before the I-485 is considered complete? Would he need to sign the I-693 and DS-3025?

Yes, you need to complete the vaccinations that are required - you can do this at a health department or any doctor - then take that evidence along with the record of vaccinations she already had to a Civil Surgeon. He will just complete the I-693 vaccination part, put it in a sealed envelope and you would include that sealed envelope with the AOS. Bring the DS-3025 with you to the Panel Physician - he will should know how to process it. Be careful - some panel physicians will try to tell you she needs the whole medical all over again - she doesn't. If they won't just do the supplemental vaccination requirement, find another panel physician who will. You can also shop around to get the best price. If all else fails, there is a doctor in Texas who will provide the necessary documentation via mail.

First of all the K-1 has NO vaccination requirement... so there was nothing needed to pass the POE... the vaccination requirement is for the green card. If she needs additional vaccinations then they can be completed anywhere... you then will take the record of the updated shots and the DS-3025 and give to a civil surgeon and they will transcribe all the history to the I-693 that you will provide with the I-485.

I do have a copy of her DS-3025 that she received before leaving Cambodia. In secton 2 "results", the box was checked "vaccine history incomplete- applicant may be eligible for blanket waiver because vaccinations not medically appropriate. Basically, she needed 2 more vaccinations after she arrived in the US. I guess I am answering my own questions, but I assume we need to get these last 2 vaccinations from a local civil surgeon and he needs to sign a form I-693? Thanks for your help......sometimes its very confusing! Thank God for VJ!

Hmm, if it says may be eligible for a blanket waiver she may not need those last two vaccinations. The vaccinations are age, gender and seasonally dependent in some cases so not everyone needs all of them, and younger applicants need different vaccinations than older applicants. If the panel physician says they are not medically appropriate, then she may indeed not need anymore vaccinations. You would then just need to get the Vaccination part of the Medical form completed and let the Civil Surgeon determine if her vaccinations are already complete or not.

Hmm, those generally are required for everyone unless for the MMR the individual has had the mumps, measles and chicken pox and acquired immunity that way. I would arrange to get those done at the local health unit as I doubt those one would be given a waiver.

Okay reading this thread has been helpful and confusing at the same time...my finance is going for her medical at the end of the month for K-1 visa...here are my questions for when we apply for I-485 after she arrives in the US...

1. In India when she at the medical, will they give her a list of vacciations she has taken?

2. What is DS 3025 and how do I get it and what do I do with it?

2. What is the list of vaccinations she needs for I-693?

3. Where do find out which civil surgeon she needs to visit here in NY before submitting I-693?

Is there a guide anywhere on this medical stuff for AOS?

Not true. This is consulate-specific. For example, Thailand requires that the applicant have all vaccinations up-to-date before the visa is issued.

K1: 01/15/2009 (mailed I-129F) - 06/23/2009 (visa received)

AOS: 08/08/2009 (mailed I-485, I-765, & I-131) - 10/29/2009 (received GC)

Not true. This is consulate-specific. For example, Thailand requires that the applicant have all vaccinations up-to-date before the visa is issued.

No, the Department of State has NO specific vaccination requirement for a K-1... I dare you to find it anywhere.... What you will find is that some consulates will try to place the immigrant vaccination requirement upon the K-1... but that is very different than a K-1 having a requirement.
