The down-loadable/fillable I-751 can be found here on VJ HERE or from USCIS's Download Site .

You can file to lift conditions any time between when the window opens (90-days before the PR/Green Card expires) and when the card actually expires - can be at the beginning, middle or near the end - it is up to you.

Be sure to review the Step-by-Step Guide on Removing of Conditions (Form I-751)

Yes you need to file form I-751. You should go to the forms section at http://www.uscis.gov for the most current and up to date form. Do you have Adobe Acrobat? If so, you should be able to download the form, open it and print it.You should file as early as possible after November 5th. Your are about to have a baby so I would get everything together so you won't have to worry about it after you have the baby (because you are going to be a busy mommy) so I recommend that you file before the babies birth.

I recommend going to USCIS.gov website and downloading the form from there. But I believe the form available there will expire at the end of October, so you may need to wait until early November to download the new/update one.

VisaJourney is a great source of information and reference, a place you can share your experience and check how others are doing with their application. But you should always use USCIS as the main website to download forms and updated instructions regarding fees, dates, documents necessary, forms, etc. I advise printing the instructions for the I-751 guide and reading it carefully. Then if you have doubts, you can search for answers here.

Also, you have 90 days before 02/05/2010 to file your application. It doesn't matter when within the 90 days you apply, as long as it is BEFORE 02/05/2010.

You can apply in December, after you give birth.

I recommend going to USCIS.gov website and downloading the form from there. But I believe the form available there will expire at the end of October, so you may need to wait until early November to download the new/update one. Nearly all the forms in VJ's Download Section are directly linked to USCIS so what is here, is the current version(s).

Also, the currently posted I-751 will not expire in October - its current version is "Form I-751 (Rev. 08/25/08)" which is the latest propagated version - the USCIS site will indicate if/when a new form is propagated and if previous versions are accepted.

Just to kind of clarify...

I didn't know they were linked. But I still think it is safer to always go directly to USCIS. But that's in my opinion.

Regarding the form, the edition is 8/25/08, but I just clicked on the form and on the top right hand corner it says "OMB No. 1615-0038; Expires 10/31/09".

OMB (Office of Management and Budget) number/date have nothing to do with the form's expiration date (bottom right of forms show version/expiration date).

As long as you're on the 90 days (3 month) window you'll be OK.

Goodluck.
