I wrote to the London embassy last week because our approval notice is due to expire on the 30th October. Unfortunately, my fiance has had some problems with aquiring the necessary evidance, and I have had to delay scheduling my medical due to a delay at my doctors (I need a letter explaining my medical condition, and they haven't written it yet), so we probably aren't going to be ready for interview until early November at the earliest.

The embassy wrote back today with the following reply:

"As it is now over four months since your fiance visa petition was approved, you will need to present an affidavit from your fiance(e) to reaffirm the petition.

This should be in the form of a sworn statement reaffirming his/her intention to marry you within ninety (90) days of your arrival in the United States."

Question: Can anyone here clarify what they mean by "need to present an affidavit from your fiance(e) to reaffirm the petition". I already included a letter, signed by my fiance, confirming his intent to marry me within 90 days of my entry into the US, but I guess that wasn't quite enough.

What they are asking isn't the "norm". Looks like all'syou need is a updated letter of intend. I'd do one from each of you and have them notarized. Hit them with both barrels firing.

K1 denied, K3/K4, CR-1/CR-2, AOS, ROC, Adoption, US citizenship and dual citizenship

Even if he included such a statement in the original petition, they are asking for *another* statement. Basically, your NOA2 is expiring and they want documentation that he still wants to continue with the petition.

A letter, something along the lines of "I, ____________, reaffirm my intent to marry my fiance(e), _____________ and wish to renew my petition to bring my fiance(e) to the U.S." and send in the signed original.

Some consulates automatically renew the petition/keep the case active, but in your country's case, the consulate is asking for a re-affirmation. Montreal Canada consulate has been super-slow and they renew automatically.

Not a big deal, it's really just a formality. It's not that they don't believe you, they just want documentation so they can keep the case active.

I wrote to the London Embassy on the 29th September, including a letter requesting an extension, (in which I also confirmed my intent to marry him) plus included copy of our NOA2, evidance to show that he visited England in June, and a letter of intent from him. The letter of intent from him was dated 26th September, so I'm a little confused about what exactly they want.

The best I can think to do is to have my fiance write another letter of intent, and this time have it notarized. Perhaps the issue is that since I sent all documents together, they want evidance to prove that I did not sign the letter of intent myself.
