Ok ive done so much research and I just don't get it.

My wife came to the US with a K1 fiance visa from the Philippines.

I read that since she's had her medical at prior, we do not need the I-693 but only the supplement I-693A.

Upon arrival to the States, all she has from St Lukes Hospital is a single CD labeled chest radiograph and with the text, " Please bring this x-ray CD to the US for medical follow-up at the Health Department.

I call many civil surgeons in my area and most do not know about 693A. I finally goto one and they want me to fill out a normal I-693 but the doctor would cross out the irrelevant since my wife already had a full medical.

Here is the problem, the doctor of course wants proof of the immunization and vaccinations. We have none. St Luke's did not provide any and only gave a CD. According to my wife, the staff member at St Lukes told her they would send all to the embassy.

What should I do? Do i need to have someone goto st. lukes in the philippines and find her immunization record? or do i disregard including the 693A since st lukes said they sent everything to the embassy? i am so lost.

Secondary note, the I-693A i pulled from ins gov site is supposedly outdated and no longer used according to the doctor thus the recomendation of using the normal I-693.

Third note, st lukes did not complete the cervical vaccination and we need 2 more shots that cost an insane amount plus influenza shot.

any insights? help? what is my best course of action.

Somewhere in your paperwork from St Lukes you should have a DS3025, this is what you send in with the AOS paperwork, the I-693 is a form to use if you want to cover all bases, it has a section on it for the civil surgeon to transcribe the info from the DS form. If the time period has not passed for you to be required to complete the 2nd shot before you file it will not be needed.

Contact the place where she got her vaccinations and see if they will fax a copy of the immunizations she received. My husband did not receive a copy of his either. Since she requires more vaccines, I would think she will need this so that the surgeon knows what she has left. I wouldn't have them fill out the I-693 vaccination portion until she has had all vaccines required for AOS.

Hi my fiance got her immunization record and x-ray cd at st lukes. On her 2nd day there.hope this helps a little

Ok i did a bit more research and she does not have a DS-3025 / immunization record. i dont know why st lukes didnt give her a copy but supposedly they sent it to the embassy. she said she saw it but they didnt give it to her and sent it for her instead to the embassy. i do not understand why they did not give her a copy. i guess il have to contact them to fax it or something but knowing the philippines and how they are there.. this is gunna be a pain. hopefully not.

now i was wondering though, at immigration at the airport, it was suppseodly included in her visa packet, the immunzation and medical record since the immigration officer told her she was a healthy girl. does this mean they have a copy already or i would still have to obtain my own copy??

They do have copies that are supposed to arrive when you go to interview. But sometimes there is no interview which is why you need to send with AOS. If it is not send you have a chance of getting a RFE then you will have to get it anyways.

It is unusual for them to not give a copy with the CD at StLukes, I remember all the paperwork my wife had when we got back together and it was all strewn together, so recheck all the papers again and hopefully it is just stuck in there someplace, if not I would get to trying to obtain a copy. Its called a VACCINATION DOCUMENTATION WORKSHEET.

if you don't find the ds3025, try to get a copy requested from st. luke's as soon as you can, scan it or send it through fedex, because w/out it showing that her medical exam was very recent, she'll be asked to do it all over again.

when you get this, she'll need to completed whatever shows in the vaccination summary needs to be completed, otherwise your civil surgeon cannot say she's medically ok for AOS.

it's correct that only one page of the medical form will be filled out by the civil surgeon, some have that form w/them at their clinics and know how what to do w/it. hopefully yours does too.

expensive as it is, the vaccinations partially given to your wife were given because she's the appropriate age for the vaccinations that's why she has to have them and complete them
