I would like to set up the interview date for the interview on Nov 11 next month. It will be the only day I can take off from work. Do you guys have an example form? Thank you for your help. And good luck with your visa process.

No such form nor example exists. Interview dates are set by the Embassy/Consulate - or USCIS, which ever applies to your situation (hard to tell since you have no profile, timeline, filed-for information nor country of origin).

I was able to request the date I wanted at the Vancouver consulate for my daughter's K-2 visa interview. I emailed the consulate and requested the date, and they replied saying that it was available.

I doubt any consulates will be open on November 11th, though, because that is Veteran's Day and I believe all government offices, banks, post office etc. are closed.

You will have to be prepared to take some time off for this visa process, unfortunately.

Good luck with that. LOL You will be told which day your interview is. You will take the day off work to be there. If you ask to reschedule, it could be several months more.
