My fiance is also a retiree and I would like to share with you the thread I started several days ago about this topic. Hope this helps.

I ask my husband about the forms that you mention, he said that he don;t have those, his reason is he is put into disability retirement.

I talked to my fiance about this and he said the documents you have now (i.e. letter from the lawyer and the information from the labor and industry) should be sufficient enough (though bank statements and asset declaration would be a big help too). Disability compensation is not taxable, thus W2 and 1040 are not a requirement in your case and you dont have to worry about tax evasion from the IRS. By the way, my fiance is a CPA and a retiree of the IRS, so I guess he knows what he's talking about. If you need further assistance, just send me a private message.
