I was not sure where to post this, so forgive me if this is in the wrong place.

Does anyone know where I can download a Pet Travel Health Certificate and an Acclimation Certficate for Dogs traveling from Russia to US?

In addition to two children, Alla brought along a dog and cat. The UNITED STATES does not require anything except a rabies vaccine certficiate for DOGS. The vaccine must have been given at least 30 days prior to travel and not more than 1 year prior to travel or the dog will be subject to quarantine for 30 days on arrival. The dog can be quarantined to your own home and have a veterinary check 30 days later. Cats and other small domestic animals (rabbits, hamsters, etc.) are subject only to a visual examination by a veternarian upon arrival.

That said, RUSSIA may require a health certificate for the animal to LEAVE. Ukraine did. Alla took the animals to the local veterinarian in Ukraine, got them both rabies shots and health checked. The Veterinarian had the forms needed. He completed the forms and sent them to Kiev and a few weeks later Alla received the official Ukrainian health certificate for the animals. She showed this when she left Ukraine at passport control. I do not remember the cost but it was not very much. She did not need to show anything except the rabies certificate when she arrived in the USA. At the POE they had a veterinarian and he came and looked at the animals briefly and checked the rabies certificate (actually just a statement from the doctor that a vaccine was given) One of the CBP people spoke Russian and translated the rabies vaccination record for him.

I would start by taking the animals to a veterinarian in Russia and have them checked, vaccinated and tell the vet you will be leaving for the USA and they should be able to help you out.

Any good vet in Russia should be able to provide you with any forms that you require. Check out the following website for specific instructions on the procedure for exporting cats and dogs from Russia--

My Natasha who is from St. Petersburg and recently took her cat (Rizhik) to the Vet and the vet had all the paperwork there. Rizhik actually got what looks like a passport with information about his rabies shot and when he got it. He also got a deworming medication before he went to the vet. There is also a requirement that the animal be chipped. Natasha has to take him back to the vet 3 days before they leave the country and then she is given a health certificate. There was some talk about some government entity where she needed to take him to confirm or not confirm that he was an exotic animal, this turned out not to be true, this is done at the airport. At the airport she has to take him 1 day before the flight to have their vet check him out and make sure everything is in order and measured and weighed to ascertain how much they will charge poor Rizhik for his plane ride. Apparently the U.S. actually doesn't even require a rabies shot to enter. This coming from my brother who is an animal control officer. The animal will be allowed to enter, however they must get vaccinated within 10 days and be quarantined (at your home) for 30 days. It is better to have everything done before the animal enters. Of course none of this relates to Hawaii or any islands held by the U.S. Well this is how I understood Natashas explanation, there are many things about Russia I don't understand (apartment registration for one) but this seemed pretty straight forward.

True, rabies shots are required only for dogs and only then if you want to avoid a quarantine.
