Medical for I-485 & other questions

I have some questions

Which form do I need for the medical? Will the surgeon have them or should I take it filled in for him to complete?

Will I need a Full medical and if so what forms is that?

Where do I find out which location to mail everything to I am in VA?

Also how do I find my local office to me?

You'll need to print out this form: http://www.uscis.gov/files/form/I-693.pdf and you fill out part 1 only. The civil surgeon will fill out the rest. Bring your vaccination record with you and they will tell you what vaccines you will need when you get there.

09/21/08- Phil arrives in US

08/15/09- Wedding

10/16/09- Mailed out AOS package

01/14/10- Interview completed - Approved!

01/23/10- Conditional Green Card received!

ROC

10/17/11- Mailed ROC package

10/19/11- Package arrives at VSC

10/20/11- NOA1 issued

10/24/11- "Touch"

10/24/11- Check cashed

10/26/11- NOA1 received

11/23/11- Biometrics appointment

01/14/12- Conditional GC Expired

07/17/12- Approved! - Notification of card production

07/23/12- Notification of card being mailed

07/25/12- 10 year GC received!

You'll need to print out this form: http://www.uscis.gov/files/form/I-693.pdf and you fill out part 1 only. The civil surgeon will fill out the rest. Bring your vaccination record with you and they will tell you what vaccines you will need when you get there.

Can anybody tell me which is the correct answer on form I-485 in section 2 answers a through h when I am applying to change status from VWP?

hi!

we had filled the form out, but our dr had his own method.. he didn't use the one we brought but instead had his own copies and typed in the part 1 for us. BUT i would have mine just in case ur dr doesn't do that..

he will do a full medical exam, administer any vaccines needed or transfer the ones u already have.. BUT the chest xrays are only needed if the TB skin test comes back positive.. so most likely that will not be done..

08/07/09 We are married!!!!!

AOS from B2 visa

09/03/09 Completed medical.. all clear!!

09/21/09 Sent off I130, I485, I131, I765.. YAYYYYYY[/color]

09/23/09 package received

09/28/2009 all 4 NOA's printed (received on OCT 1ST)

09/30/09 checks cashed!

10/05/09 receive biometrics appt for 10/28/2009

10/13/09 walk in biometrics.. !

10/26/2009 receive interview letter for NOV 30th 11:30!!!! PRAISE GOD!

11/06/2009 receive advance parole papers.. no email, no updates beforehand..

11/16/2009 After 2 WEEKS of checking the mail thinking the EAD and AP come about the same time, we get the "card production ordered" email for the EAD.. better late than never.. thank you LORD!!!

11/19/2009 receive another email stating "card production ordered"

11/23/2009 receive EAD!!!!!!!!!!!!!!!!!!!!

11/30/2009 bittersweet interview, but WE ARE APPROVED

12/02/09 social security card in mail

12/04/09 receive "welcome" letter in mail

the skin test requires u come back in 2 days for them to check the reaction.. i was told from there they decide if the xray is needed..

As you are just ahead of us having actually filed!!

You maybe able to answer my next question as it seems you are applying for similat aos.

I have heard mention that we have to supply a clean police report from his home country UK?

Is that correct and if so when do we provide it? Or is it not required applying AOS?

He has a clean record so has nothing to worry about we just do not know if it is needed and when?

For the I-485, police clearance is only requied in certain situation... read the I-485 instructions for more info. If it is required, then it is part of the initial evidence.

I just had a look at i-485 and from what I gather we do not need it?? He has never been arrested or convicted?

Does that sound correct?

The only thing is it refers to i-485 additional form that I can not find?

Any ideas?

Most of the time, you will do a background check/police clearance, is when you are going through the K1 Fiance Visa process (consular processing). When you adjust from VWP, you don't need to have it. You can if you wish, but its not always required.

What needs to be done is a concurrent filing of I-130, I-485, I-765, with the I-864 from the sponsor. Make sure you also have the G-325's for Biographical information. You can file for Advance Parole, but if there is any overstay involved from the time you enter this country to when you do your filing, I would not use it. When you contact the Civil Surgeon for an appt., ask them if they need you to bring in the 693 form. The one we used, had their own and wouldn't use ours.

Well, I guess I had the perfect doctor- I looked him up using the USCIS website.

I didn't need to bring any form (I-693) or vaccination reports. He just asked me some general questions and he took a blood sample for testing.

The form was filled out by him, he just had me checking if everything was correct. It did cost around $300, but he didn't give me any headaches.

My N-400 Journey

06-02-2017 - N-400 package mailed to Dallas Lockbox

06-06-2017 - Credit card charged; received text and email confirming that application was received and NOA is on its way

06-10-2017 - Received NOA letter from NBC dated 06-05-2017

06-16-2017 - Received Biometrics Appointment Letter for 06-28-2017

01-19-2018 - Interview Letter sent

02-27-18 - Interview and Oath Ceremony. Finally US CITIZEN!
