According to the Mumbai Embassy, my fiancee's Packet 3 was mailed out to Pune on Sep 4th but it never arrived. I called the Embassy on Sep 22nd and they said I can just download the packet from the website. However, without any clear instructions on the documents I downloaded from the Embassy website, I'm confused about what to actually send back. See more information below.

  • I am clear that DS-230 Part I has to be sent back
  • I am NOT clear if I need to send back DS-2001, Applicant Statement or both
  • If Applicant Statement needs to be send back, do I include all the pages (including the checklist) or just the last page. Remember, the Applicant Statement is a continuation of the checklist and not a separate page/document.
  • On the Instruction package for K-1 visa applicants (checklist), it says 'check the box before each item and return this form to the Immigrant Visa Section at the American Consulate, Mumbai'. If I have to send this back right now, then I'm assuming I have to leave the medical question unchecked because I need an appointment letter to complete medical and obviously I don't have that yet. If I have to send this later, then do I do that after the medical but before the actual interview?
  • In addition to the items mentioned above, do I need to send back anything else as a response to Pakcet 3?

you just need to send form DS-2001 and Applicant Statement. just send the applicant statement which is like a paragraph.

nope you don't need to send anything else besides these two for now. and then wait for packet 4.

my fiance didn't receive his packet 3 either and we called embassy and got the same instructions. My fiance filled the forms out and then emailed it to embassy and they replied same day saying he should receive his packet 4 in 2-4 weeks and which he did :)

actually we emailed to embassy..so I wouldn't be able to tell you the actual mailing address. you could call back embassy and ask them.

You only send the DS230 and the 'Signed' Applicants Statement, not the check list.

Again, you send 'only' the DS-230 and the signed Applicants Statement, Not the check list. The check list is only for your convenience to show you what you will need before they can schedule you your appointment. You check these boxes as you acquire each doc and then check it off as you do. The Applicants Statement and your (the benificiary's) signature is telling the Embassy that you now have 'ALL' the required documentation and they can complete processing of your case. You'll get the interview letter soon after informing you of your interview date.

So, make sure you have all these docs ''before' you submit theses two forms.

August 10, 2009- Embassy notified via email that they've received the Re-affirmed petition assigned with a 'new' case number as well as mailed Packet 3.

August 20, 2009- Embassy sent as an email attachment, the DS-230, Applicants Statement and Cover letter to Packet 3.

August 20, 2009- Mailed and Emailed request letter to Embassy to extend validity date of petition.

August 25, 2009 - Embassy received request to extend validity letter.

Sept 2, 2009- Embassy confirmed their receipt of the completed DS-230 and Applicants Statement.

October 29, 2009- 2nd Interview date- Visa Approved!

November 4th, 2009- Received Passport with Visa intact!

December 4th, 2009- Fiance arrived in U.S. - MARRIED Dec. 7th!

February 23rd-Mailed AOS Packet

March 3rd, 2010-Check Cashed

March 6th, 2010-Received AOS Receipt Notice

March 29th, 2010-Biometrics

Sept. 14, 2010-GC Interview -Approved!

My paperwork is on it's way to Mumbai Consulatenow and hopefully we will get our Packet 3 and interview date soon.

You only need to send back two things:

What address do I send these 3 documents (DS-230 part I, DS-2001, Applicant Statement) to? Is it the standard Mumbai embassy mailing address or is there a special department within the embassy handling this? Could you provide the address that I need to send this information to?

I'm wondering how anyone was able to email these forms to the embassy? Did you sign it first and then scanned the copies in? Is that allowed? I thought the embassy wants originals of everything.
