Question about G-325A

I don't have a ton of time right now and so this is going to be kinda short but I hope you get what I'm asking.

Form G-325A asks for "Mother (Maiden name)" right? Well my step-mother is the only person that I consider to be my mother but of course that name would be different from the name of my mother on my birth certificate. Biological mothers maiden name is "Price" but step mothers maiden name is "Self". I thought about just sending the copy of my passport as proof of citizenship that way it won't be confusing but I used my birth certificate to get the passport and so that would be on record. I'm not sure what to do so any help would be wonderful. Thanks in advance.

Yeah, I was afraid of that and if I were to be honest with myself, I already knew that too but just didn't really want to have to re-do the darn thing. Also part of the problem is I have no idea her birth date, city and country of birth (which admittedly it says "if known" so I'm ok with that one), or her city and country of residence. I suppose there are orphans that have this same type of problem, I just have never ran into this sort of question before. At least not one where I couldn't explain around it.

So for date of birth, city/country of birth and city/country of residence do I just put "unknown"?

For both of my birth parents I put "UNKNOWN" for their names, dates of birth etc. While I do know their names I had no legal proof; they were never listed on my birth certificate, I wasn't raised by them and I was never legally adopted by the "parents" that did raise me - hence officially I had no parents. I was worried about this throughout the process from the time I sent in the I-130 right through to the interview. It was never an issue and was never brought up at any point. You should be fine with "UNKNOWN."
