Since we all pretty much have to know more about what's going on than anyone who's assisting us in this endeavor (see this thread), how does the civil surgeon fill out 1-4 of Part 2 of the I-693, when he is only transcribing vaccination information? My wife's medical was in July, so she doesn't need another one, she only needs the vaccination information. We don't have a copy of the DS-3025 from the medical; I guess that's in the mystery envelope we handed over at the POE.

My wife's medical was in July, so she doesn't need another one, she only needs the vaccination information. We don't have a copy of the DS-3025 from the medical; I guess that's in the mystery envelope we handed over at the POE.

Double check to see if she has the DS 3025 record of vaccinations. Says DS 3025 in small print at the left side bottom of the paper.

Double check to see if she has the DS 3025 record of vaccinations. Says DS 3025 in small print at the left side bottom of the paper.

We have no DS-3025, our only copy was handed in along with everything else just prior to her interview. Anyway, she got her vaccinations in China after her interview, as they were not required for the interview itself.

Did they give you vaccination record certificate or letter from the clinic in China? and chest X-ray? Yes, at POE they give you your case with all docs. there should be your medical examination record. but still they will ask you to show them your medical vaccination record.

We got records of the shots she was given in China in a neat little book, not a US form, and we have her X-ray, as the guy at immigrations told us he didn't need it. We did not get anything from the mystery envelope back from him.

My question is, what do I tell the civil surgeon, who knows less than we do about this process, to do with the questions on the I-693 pertaining to the medical, when he is not the one who performed it, and I have no records of said medical, except a receipt from the clinic?

Hi, my fiancee and I also have same confusion re: the I693. As Kevin and Lei Lei, my fiancee didnt have the copy of DS 2035 because it was sealed in the brown envelope and given to the immigration officer at POE in LAX, but he does have the copy of his CXR and Immunization record.

I did try to call civil surgeon around my area to transcribe the record in I-693 form, and we ran into the same problem as Kevin and Lei Lei, they didnt have a clue and most of them said he had to do it all over again and they are very costly.

I tried to call Dr L.E. Arnold and left a message but hasnt gotten any respond from his office. Is he really legit?

so confused.... so the question is if the K1 visa holder already had physical exam and up to date immunization, and the docs already given at POE to the immigration officer, does the K1 still need to include I693 or transcribe the immunization record to I693? and file it with AOS??????

Couple #1--you got shots after the official medical exam so those would not be on the DS3025 signed for by a US approved physician. The USCIS needs to have one of their official approved doctors sign off on your immunizations. Since it didn't happen in China at the medical exam, you MUST have it done by a Civil Surgeon in the US. I suggest you send your things to Dr. Arnold in Texas to do it by mail. He will look at the immunization book from your country and fill in the form. He doesn't have to see something from a US approved doctor. He looks at what you have and tells the USCIS if it's okay. They aren't doctors so want a doctor (US Civil Surgeon) to approve the record if it didn't come from the Embassy doctor. You can show Dr. Arnold any record...childhood records, the neat little book from China, or a shot record from any clinic in the US. He will put it all together on one form and sign that all immunizations needed are done. The Civil Surgeon only does the immunization part (page 3). You can't tamper with the blank parts because he seals it in an envelope which you must send in still sealed.

Couple #2--I hope that helped answer your question. If all your shots are up to date and on the form signed by the Embassy physician, then it is possible that USCIS will accept it straight from the brown envelope turned in. You are different from Couple #1 who got shots after the official medical and so their form is not complete even though they have all shots now. You can also take whatever records you have to a Civil Surgeon to be put on an I-693. Dr. Arnold calls back...everybody has to leave a message on the answering machine. I answered your PM too.

Your Form I-485 is filed within 1 year of your overseas medical examination

If you did have a Class A medical condition, you received a waiver of inadmissibility and you have complied with the terms and conditions of the waiver

Even if a new medical examination is not required, you still must show proof that you have complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original, overseas medical examination report, you will have to have the vaccination report completed by a designated civil surgeon. In this case, you are required to submit Part 1, Information About You, Part 2, the vaccination chart, and Part 5, the Civil Surgeon's Certification, of Form I-693 (in an envelop sealed by the civil surgeon). Please see the instructions for Form I-693 for further information.

I just realized the civil surgeon I visited didn't give me a record of the two shots he gave her, other than page 3 of the I-693 that he filled out. I hope that's an acceptable record, if I decide to use Dr. Arnold.

I have a similar issue and my time is truly running out. I had my medical in the UK in Feb. I needed to get some shots but the doctor said I'd be cheaper doing them at my local practice. Which I did. I never got a DS-3025 either as I believe its in the envelope. But from what I've just read, they wont have my up to date record of shots. I do however have a print out of all my vaccinations from my local practice which I was going to send in as part of my AOS packet. Will this be accepted?? I flippin hope so as I've literally left all this to the last minute, I should have sent of this darn packet months ago. Anyway, I hope this will do as I don't have time to deal with Dr Arnold. Aaaaah.

Big-time CONFUSION.

At what point in the process should I have seen the DS 3025? Like Kevin & LeiLei, I can only assume that this was included in the sealed "mystery envelope" the immigration officers got at POE. We have NO RECORD (from either childhood or adulthood) of immunizations/ vaccinations, NO DS 3025, NO COPY of the medical exam...so how are we supposed to handle the I-693/ I-693A issue? Can we use Dr. Arnold's services? Or will we have to pay for vaccinations here in the US (as the below-quoted USCIS instructions imply)?

The guides are not (in my opinion) clear on this in that they refer to a "copy of the medical check form" that I've never seen. Where is this copy?

Most K1's like myself didnt get a copy of the vaccinations my medical records were sent straight to the embassy in my home country when i had it, so i sent my AOS without the form so far i have received my AP and my EAD im now awaiting on my greencard i havent been asked for the vaccination record as i put on my cover note that it should already be there, as it was in the sealed envelope i handed over at POE! best of luck hope this helps
