I filed for a k-1 visa with my fiance. I just received the forms and checklist and find myself confused.

In the acceptance letter it said that my fiance and I were to be married before January of this year. In the cover letter for the forms list it says " upon receipt of the above- listed documents, this office will schedule a visa interview for a date approximately four to six months after the receipt of the required documents."

Now how can it be that I am to be married by January but after I send in the requested documents I have another 4 to 6 months wait?.

Also, do I have to set up my own medical appointment date or am I given one?

The checklist says I need to have a form saying I have had the medical.

In all my research I was told I would be sent a date for both medical and interview after I sent in the required forms listed on the cover sheet.

Thank you for your time and help

Who sent the "acceptance letter" and checklist to you?

What country is your fiance from? What consulate will conduct the interview?

In the acceptance letter it said that my fiance and I were to be married before January of this year. This seems strange. Are you sure you aren't misunderstanding it? It might be helpful to post exactly what the letter says. In the cover letter for the forms list it says " upon receipt of the above- listed documents, this office will schedule a visa interview for a date approximately four to six months after the receipt of the required documents."

It would be very helpful to know what consulate you're dealing with. It sounds as though your I-129F petition has been approved and sent to the consulate, but we have no way of knowing that because you haven't filled in your timeline. So... :time: -> http://www.visajourney.com/timeline/intro.php

Improved USCIS Form G-325A (Biographic Information)

Form field input font changed to allow entry of dates in the specified format and to provide more space for addresses and employment history. This is the 6/12/09 version of the form; the current version is 8/8/11, but previous versions are accepted per the USCIS forms page.

Be prepared to wait 3+ months for an interview.

Montreal is super slow.

Okay, this is very helpful information.

First of all, the Approval Notice states a validity of four months from the date of approval. Don't worry about this. It has nothing to do with when you're required to marry your fiance. The consulate is aware that your approval will expire before the interview. They have the authority to extend the validity. The US fiance will have to submit a new letter of intent to marry (possibly notarized) at the time of the interview. With this letter, they will extend the validity.

Questions about the medical exam are consulate-specific. Now that you've identified the consulate as Montreal, people with experience there will be able to help you.
