Marital Status on ID, Police Record and Passport

Hi, everyone:

My wife and I were married in May 2009. Our I-130 and I-129 applications were recently approved. We are working on preparing our next set of documentation to send to the NVC for the visa application. I have read that we will need to send is her official police record. Our 3 questions are:

1) Being that we were recently married in May 2009, she has not gotten a new driver’s license with her marital status updated on it. Using that drivers license/ID in El Salvador, the police record would state that she is single. Is that OK? Or does she have to go get a new updated driver’s license so that the police report would state that she is married?

2) Same question, but for her Passport. Going into her interview at the U.S. Embassy in El Salvador, does her passport need to reflect her current marital status, or is it OK if her passport still says that she is single? Does she need to get a new passport?

3) If my case has already been transferred to the NVC, do I need to download, fill out and send in a Green Card Application? I see they are available on the NVC website: http://www.usimmigrationsupport.org/greencard_marriage.html
