I just received Packet 3 (yay!) in the mail recently, and have now begun the process of going through it. However, I totally feel confused and since I couldn't get the answers I needed, I thought I'd come to all you for some help. My questions:

1. After reading the checklist, it looks like my first step is to send in DS-0230 Part 1 by fax to my Canadian consulate -- however, do I just send that itself OR am I suppose to send my checklist, too (even though I have not gotten my police report + birth certificate yet)?

2. Speaking of DS-0230, some questions about filling it out:

a. Number 14 - 17 speaks about a spouse. Is that part where I fill in my fiance's information? Or do I just leave that park blank?

b. Number 18 has a date of marriage. We were planning on having a civil wedding as soon as I get into the US -- however, do we have to put a specific date in or something else?

c. Number 20 - 28 : that IS about my parents, right? Not my fiance's?

e. Number 33 - does that include elementary school? And technical college, too?

f. Number 35 - I've been to the US so many times, but when they talk of previous visits, how far back should I go? I can remember up to maybe three years ago, but other than that, I really can't remember. Also, I'm planning a day trip in October to go shopping with some friends, should I add this to the application as well?

I know this is a lot, but I couldn't seem to find the right information in the forums after looking! Thanks so much for your help in advance :)

Improved USCIS Form G-325A (Biographic Information)

Form field input font changed to allow entry of dates in the specified format and to provide more space for addresses and employment history. This is the 6/12/09 version of the form; the current version is 8/8/11, but previous versions are accepted per the USCIS forms page.

I'll answer what I can:

I've seen different schools of thought on this; some people include high school, others start with post-secondary (college, university, etc.). Either way, technical college should be included, and start with the most recent and work backward.

Thanks so much for your help! Those forms definitely help me out.

Now I just need to find out if I'm suppose to send DS230 Part 1 out on its own OR together with the Checklist.

Also, has any of you completed everything on the checklist and then sent it, or just sent the checklist with things in pending (like, waiting to get my police report + birth certificate, etc.)
