DS 157 form from bank

When i paid the bank they give me a receipt attach to a blank DS 157 form.

Do i still have to fill it up?

DS 157 is required of male applicants over age 16. Otherwise, no.

garyandalla, i had to fill one out and i was way over 16 yrs old then and very much female

anyway, masterific, you need only 1 of that so if you already have one filled out, that one is ok, no need to fill out the one the bank gave you.

That's Bullsh!t!! Maybe in other countries but you HAVE to fill out and submit a DS 157 no matter what age and sex you are.

I stand corrected for the Manilla consulate. Kiev requires it now for males over 16. Ok, thanks Haole.

Yeah I try to stick to advice only on the Philippines as each country has different procedures. After peeps get to the states it's pretty much the same tho.
