Hey, may I ask for your opinion about the other names issue?

Due to my mum's divorce and remarry, my name was changed twice. First time, 3 years old, changed to follow my mum's family name after she divorced. Second time, 12 years old, changed to my current father's family name after she remarried. Since 12 years old, never changed.

In this case, whether I should input both previous childhood names on the form 230I in P3?? I don't want to make things complex and make the background check longer...

By the way, my bio-father has passed away. Whether I should also prepare for his death certification?

24 May, See NOA1 online.

6 June, Received paper P1

21 Aug, See approval on website.

28 Aug, received paper P2.

31 Aug NVC mailed out to consulate.

18 Sep, Get phone call from consulate, confirm about mailing address.

25 Sep, received P3.

25 Sep, Send back P3.

28 SeP, Consulate received P3.

20 Oct, call DOS and got interview date: 19Nov

25 Oct, P4 received.

3 Nov, Medical Check

11 Nov, Medical Check Report Received and pick up my bf in airport.

16 Nov, My bf attend petitionor hour in consulate

19 Nov, Interview approved.

22 Nov, Visa package arrived.

7 Dec, Fly with my bf together back to US.

What's a 230I? Are you talking about the DS-230? If so, in answer to question 2 you would list any names you've ever used, including legal names and aliases. Don't list your current legal name - that goes in question 1. The whole point of them asking about other names and aliases is so that they can do a background check on those names. If you fail to list a name just because you "don't want to make things complex" or "make the background checks take longer" then you risk a finding of material misrepresentation - your visa would be denied, and you'll face a possible lifetime ban from the US.

You don't need to submit death certificates for your parents, brothers, sisters, or practically anyone else. The only death certificates that might be needed is if you had a previous marriage that ended because of the death of your spouse. In that event, you'd submit the death certificate rather than a divorce decree.

08/18/2010 - AOS Interview - APPROVED!

05/01/2013 - Removal of Conditions - APPROVED!

Form field input font changed to allow entry of dates in the specified format and to provide more space for addresses and employment history. This is the 6/12/09 version of the form; the current version is 8/8/11, but previous versions are accepted per the USCIS forms page.

So if she has me call her by a different first name when I communicate with her, rather than by her Chinese first name she would have to include that? (she has me call her this other name because she doesn't like her Chinese name and would eventually at some point like to legally change her name to this other 'americanized' name),

If it's just a name used between the two of you, I wouldn't worry about including that.

If she's ever used it on paper (signing documents, applying for jobs, credit, etc.) it should be included.
