AOS application question - I693A

Hello everyone,

I am in the process of getting all the application together for AOS and hope to use this place to type in my questions -

My first question regarding the infamous 693A (I know its been asked a million times, though the answer always evades many of us!)

I downloaded the form from CDC as indicated somewhere on this forum at http://www.cdc.gov/ncidod/dq/pdf/ti-03/appdx-a_693vacc.pdf - - however, I was lucky when the civil surgeon's office accepted to transfer my vaccination details onto 693. However, when I went there, the lady informed me, that the form from CDC was wrong, and she is aware of people being sent back... so, she advised me to print out I693 from the Kinkos close by, which I did.

And so, they took Page 1 (my details) Page 3 (Vaccination details) Page 5 (Civil Surgeon details) - - they transferred all my information on this make-shift form (gave me a photocopy), signed, stamped, and sealed it in an envelope, which they happily handed me after charging $30. Though I was happy that was done, now I am having doubts -

1) Do you think it is ok? Just 3 pages of the actual I693? Or should it have been the CDC form?

2) Also, now I see on the photocopy, that the I693 that I printed out from the USCIS site, says valid till 08/31/09 on top right corner!!! Though on USCIS website they say - Edition Date :06/05/08. Medical exams conducted on or after August 1, 2008, require use of the 06/05/08 edition - - on the bottom right, yes, my form says 06/05/08!!!

I am totally confused, and frustrated, can anyone help me please???

The CDC form I693A is no longer used. It was deprecated as of June 2008. The vaccination section of the I693 is what you need (along with the personal info section and the Civil Surgeon info section). So it looks like you got the right stuff. As long as you got the latest form for the USCIS site, you are ok, don't worry about valid until dates.

Anyone looking for a local civil surgeon from the Raleigh Durham region, please do contact me - Not sure if I can post the name and contact details here - - the doctor's office was really helpful, and when I called them the first time to speak of my case, they just said, get those forms in and we can see what we can do .. Yes, I didn't have to do the complete medical, just the vaccination details transfered. Not sure if this doctor takes out of state application though. If the admin or moderator or old members say ok, I can post the contact details of this doctor.
