Richa,

India & Pakistan pretty much share the same culture in so many ways, including a professional environment. You dont need to get an affidavit format. Just have somebody sent to the local court area where there are people sitting outside or in cabins and they already have such formats. They'll charge you a nominal fee and print out the affidavits on government issued judicial stamp papers. Make sure you get the english verison and not the Hindi one. That way you wont have to worry about the translation.

i think i don't understand what you mean but let me try. you mean in india outside the court there is some people sitting. so i have to tell them and have write for me. is that correct? because i have another chance is my dad's elder sister is there and if i sent her a letter written in english she just need to sign and same for my mom too.

Yes Richa,

There are people outside the court premises( Lawyers/Munshi's/Oath Commissioners/Notary Public, who would do it for a fee. Its more convenient for you that way.
