On I-129F:

10. My citizenship was acquired through...(my fiance checked "Birth in the U.S.")

Have you obtained a certificate of citizenship in your name?...(We don't understand that question)

15. Your fiance's address abroad...where it says "Phone number...include country, city and area codes"...(What do they

want here? We're not clear on that one. Is it just 1-XXX-XXX-XXXX?)

18. Where it says "explain the circumstances under which you met"...(Do we write exactly what we wrote in the "How

We Met" letter? Or do we simply attach that letter?

Notice that it's under a box marked "Parents". The box labeled "Have you obtained a certificate of citizenship in your name?" only applies to people who have a claim to citizenship through their parents; e.g., they were born abroad to one or both parents who were US citizens. These people may, optionally, choose to get a certificate of citizenship. If they do, USCIS wants information about the certificate.

15. Your fiance's address abroad...where it says "Phone number...include country, city and area codes"...(What do they

want here? We're not clear on that one. Is it just 1-XXX-XXX-XXXX?)

For Canada, yes, though the "1" prefix is superfluous. For other countries, a complete phone number dialed from the US might look like 011-cc-xxx-xxx-xxx, where "cc" is the country code.

We Met" letter? Or do we simply attach that letter?

Give a brief one or two sentence explanation, and then say "Refer to attachment xyz".

If citizenship was acquired through birth in the U.S., nothing further is required for this question. The question about the certificate of citizenship, you'll notice, is directly underneath the option for citizenship acquired through parent(s), and is only applicable to that case.

Yes, if the beneficiary lives in Canada, it'll be 1-XXX-XXX-XXXX.

No letter is required. You must simply explain the circumstances under which you met in person within the last two years. If you need additional space, you would write "See Supplement" and label the additional sheet accordingly.

Improved USCIS Form G-325A (Biographic Information)

Form field input font changed to allow entry of dates in the specified format and to provide more space for addresses and employment history. This is the 6/12/09 version of the form; the current version is 8/8/11, but previous versions are accepted per the USCIS forms page.

10. If you are a Born-here citizen then he did not obtain an "certificate" other than his birth certificate. The second part is is NA for a birth citizen

15. If your fiance calls you from the USA, write down the numbers he pushes. 1-514-XXXXXXX There is no "country code" or city code when calling Canada from the USA as we use the same phone systems

18. Explain how you met in person. One or two sentences should do. " We met online and corresponded be email and phone for 8 months, then I flew to Canada on (date) and spent 4 days with (name of fiance) in Winnepeg" for example.
