A civil surgeon (a USCIS designated doctor) may be required if you need to update your immigration medical information AFTER the alien has arrived in the USA.

All K-1 applicants will need to visit a panel physician (a Department of State designated foreign physiscian) for a medical examination BEFORE a visa can be issued.

They are also used for AOS if the original medical exam is over year old or if the USCIS requests that the I-693 is filled out and sent to them. Most people file for AOS before their medical has expired and only send a copy of their vaccination worksheets and they're fine with that, so there's no need to get a civil surgeon involved. But, the USCIS requests the I-693 to be sent as well from time to time, so you never know.

Ours was very polite and very nice. He pretty much affirmed that apparently, somehow, my wife had forgotten to grease the doctor's palm in Tashkent.... I am not sure if the civil surgeon even charged us. There are some people in the health care profession who are nice, and professional.
