Cost for vaccine supplement?

Here's the story. K-1 that has had the medical less than one year ago. We have the vaccine worksheet DS-3025 that was given at the overseas medical but it doesn't have anything filled out besides the name and personal data stuff. So I guess we need to have a civil surgeon fill out only the vaccine portion of the I-693. We have her vaccine history from her country. Correct me if I'm wrong, but I think I have that part down.

So how much would one expect to pay to have a civil surgeon do that? I want to know that I'm not taking one up the you know where to get this done. My fiance called a place today and I don't think they really understood what it was exactly that we need. Nor does she really understand. They told her $300 and that can't be right. I assume that is the cost for the full medical and filling out the entire I-693. Also, she may need a couple of shots so does anyone know or have advice where to get those done. I would assume the civil surgeon would charge an outrageous price but that is only an assumption. Do most large cities have places to get vaccines for a lesser price. I hate seeing doctors so I don't know much when it comes to shopping around for medical bargains.

I'm in the Omaha, Nebraska area if anyone has any knowledge or advice specific to here.

What happens if shots are required? Will he just say that everything is fine? I like the price but it seems like a bit of a hassel if she needs to get shots. Also, does he do all of this through phone, fax and email? I only have about two weeks to get this done so sending everything by mail seems like it might take a while. Thanks

go to your local health department and get them done... then provide the info to the Dr.
