Form DS-230 (and I-134) sent by consulate in Packet 3 is expired

Hi folks,

So...I just noticed that the form DS-230 sent to me (us) by the Naples Consulate its very own self in Packet 3 is expired. At the top it says "Expires 05/31/2004". This is true also for the I-134 (they sent me the one that expires in 2007 when there is a 2010 one available). What do I do? I'm assuming just fill out whatever ones they sent me?

RATS! I've already almost finished filling out the other one. Oh well...I'd rather have my paperwork be current, so it's worth filling it out again. Thanks!

Here's a hint: you won't find DS forms on the USCIS website—they're Department of State forms.
