I paid the visa application form in BDO (Filinvest-Alabang branch) today. The lady in the bank told me that it was their first time to have a client bringing completed DS-157 form and she insisted to provide me a blank form because they cannot guarantee me that the USEM will accept my form. I still asked her to just do machine validation on the DS-157 form that the attorney provided us. She validated the form I brought but she just asked me to sign a paper stating that I cannot refund my payment if the USEM refuse my DS-157 form.

Does anyone ever tried bringing completed DS-157 form in BDO for verification? Is it acceptable in the USEM?

I think it is just similar forms (downloaded online/form provided by BDO) and my fiance told me that it is better that I have my computer typed form validated rather then getting the blank form from BDO and filling-out the form by hand-writing because mine is more readable.

Thanks for the fast reply. I was just a little worried lang because I don't want any delay during the interview if ever the USEM will not accept the form. My fiance will be here for my interview this September 22 ( he will stay for only a week) and I just want to be sure that we will be able to finish it on the scheduled date. Thanks a lot!

i have a copy of a blank DS157 which i downloaded from the website plus another one from BDO when I paid for visa application bcoz USEM needs 2 copies, the one from BDO has machine validation at the back , the one that I downloaded donot , both submitted to USEM.

good luck on your interview.

I had Meryll attach the blank payment validation DS-157 form to one of the two I had filled out on the computer. Many other VJers have done this before me.

Just because the majority fill out the blank payment validation DS-157 form by hand does not mean it is the only way or even the proper way.
