I am trying to get my wifes AOS package together. She has not had any of her vacinnations. I contacted the closest Civil Surgeon using the locator to try and get a price quote on her shots. When I asked for a quote, the Civil Surgeon asked which ones does she need? I said all of the required ones. His reply was which ones? I reiterated the required ones for her AOS to get her green card. He seemed confused.

So I went to USCIS site and got this list: My wife is 23.

Rotavirus

Hepatitis A

Meningococcal

Human papillomavirus

Zoster

Is this the correct list? I found a copy of the DS3025 and there are alot more than this on that list. I was under the impression the Civil Surgeon would know, but he seems clueless. I was also told by that Civil Surgeon that my health insurance won't cover the vacinnations which is BS. If they are under $200.00 then I will just pay to cut down on the chance of a mishap. If they go over $200.00 then I will get my family doctor to administer the vacs. using insurance and use the Civil Surgeon everyone else on here uses to transcribe them. I just want to make sure I get her the correct vacinnations. She had her medical done in her home country prior to the Visa being issued.

Tdap, MMR, HPV(3doses), and Varicella(2doses).the Civil Surgeon should know what are the required vaccines,..i had my 1st and 2nd dose of HPV in the Philippines and also the varicella. when i got here in th US, i went to the local health department for my 3rd dose coz its cheaper than to get it to the civil surgeon. why is she not allowed to be vaccinated? allergy? well, Goodluck to both you...

Unfortunately, it will likely cost you more than $200. A single dose of HPV shot is around $180 and there are 3 of them taken over several months. For AOS purposes however, I think you would just be medically required to have the first shot in the series completed.

According to the information I was given at my immigration medical, females in our age group (I'm 21, so similar to your wife) are required to have the TD shot or booster, the MMR vaccine, the Chickenpox vaccine (or verbal declaration she's had chickenpox), and the HPV. There was never any mention of the rotavirus, Meningococcal or zooster.

All we want is to get / do everything what the USCIS ask/want but not spending so much money :).

In vaccination worksheet, its actually check what are the vaccines she still need to take. As far as I know you just need to follow that report, coz USCIS will look on that DS-3025 and they will based the I-693 if necessary if she already completed the vaccines in Phils & here in USA. I am just wondering why she did not have any shots in PI?

But to help you a little bit, we used Dr. Arnold to transcribe my vaccination report. I took varicella here in USA, (1st shot was in the Phils but because of insufficient time interval, I did the 2nd shot here ). We sent the DS-3025 & the immunization record given by my doctor here. But before I got that shot I need to spent $75 for the shot & $125 for my doctor coz they said that I don't have a record here so I need to do a check up first before they do the shot. Dr. Arnold fee is only $30, most of the VJ's used him to transcribed there I-693 too..

The cheapest way to get the shots is at your local county health department. They will give the shots & give her a shot card to show to whoever needs to know. You can take a copy of the form that you have to them & they wil stamp & sign it. Sometimes they are not familier with the form so you may have to explain it to them.

We did it this way & the cost was 23 bucks. The info was accepted by USCIS with no problem. We have used the shot card when she went for some job interviews.

A civil surgeon should know. Ours was very helpful. Gave us a list of vaccines my wife needed based on age and sex. Then we looked at what we had based on the DS3025 and got our family doctor to admin the other ones she needed. Then we got them transcribed by the same CS for $30

I would call another civil surgeon, one who has actually done this before.

hi there! i suggest you go to your local health department (that's what i did) and show her vaccination supplement. whatever she needs more will be administered to her. when i did that i was only asked to pay less than 20 dollars but of course our health insurance covered for it. as soon as i had mine completed, i called dr. arnold and arranged things with her too. now we're all set to send the AOS.
