Arch and Cel,

Where are you getting the information about a notorized I-134 form and 3 years tax returns and W2s. On the instruction sheet for the I-134 it states a notorized I-134 is not needed and only stipulates 3 years tax returns if self employed. I sent my fiancee only last year's tax return and W2 and just a signed I-134. I followed the instructions. Did I overlook something?

Hi There,

Im glad your fiance passed the medical examination.

I just want to inquire about the the immunization record. I may have my medical exam late this month but my mother cannot provide me my record ( she kept it when we were young) because it was along time ago.I also doubt if the hospital where I had my immunization has the record. I read in the US embassy in Manila that if I dont have my record, the doctor and I will work on what immunizations I can have.

I am thinking of going to the doctor and have some of the immunizations so I do not have to spend too many days at SLMEC just having my immunizations.

Then i could have my new record to be given to the panel doctor.

What do you think?

Please help me guys, I need your advice or any idea on this.

Thank you.
