heh, I got 3. My lawyer got one first. It was white and looked like a fax/copy. I filled it out and sent it in. 3 days later my lawyer got another one. This time it was blue and looked more "official" looking. He sent it to me overnight and I sent that one in also. He said it couldn't hurt in case the first one wasn't right. When I was coming back home from Fedex after sending the second one I checked my mail. A THIRD one!! I called the lawyer and he said to keep it as a memento.
