USCIS form I-693 expires 08/31/09

Hey, have any others who are going through AOS noticed that the USCIS medical form I-693 is set to expire next week? Aug 31!

I'm about to fire off the DS-3025 to Dr. Arnold in Texas, and this caught my eye.

Since by the time we file AOS it will be Sept, this form will be technically expired. Any worries here?

It's at the top right hand corner of the form - OMB No. 1615-0033; Expires 08/31/09

This is the "new" form (which has the vaccination list inside the document itself, rather than as I-693A supplement.

At botttom right it is marked Rev 06/05/08. So it's only been in circulation for the past year.

Since there doesn't seem to be a newer form I suppose this is the one we need to use?

The (top-right) OMB date is irrelevant (for expiration date purposes).

Technically the form is already 'expired' - no new form has been propagated. The Edition Notes explain:

"06/05/08. Previous editions will be accepted only for medical exams conducted before August 1, 2008. Medical exams conducted on or after August 1, 2008, require use of the 06/05/08 edition (link at bottom of page)." (The link points to the currently posted I-693.)

Whatever Form is on the State dep website wilo work even if expired. The DS 157 was expired and people used it for months without any problems.

I imagine the same goes for the DS 693.

for as long as there is now new/updated version of the form on the official uscis website, that form is still good to use
