K1 AoS Vaccination Supplement

Hi,

I arrived in the US just over 2 months ago on a K-1 fiance visa, and recently married my fiance. I now need to apply for my adjustment of status. I have looked at the Step-by-Step guide on here, which I found VERY useful and now almost have all my ducks in a line. The only outstanding issue I have is the Vaccination Supplement.

According to the I-485:

"If you are a K-1 fiance or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiance visa, you only need to submit a vaccination suppliment, not the entire medical report. You may include the vaccination supplement with your Form I-485"

Now my questions is, what is the Vaccination Supplement? Is it

a) My DS-3025

B) The I-693A

c) The I-693 (partly completed)

I have looked on the USCIS website, and there is no I-693A form to download. I did however find this somewhere else on the web. Another forum said that the Civil Surgeon should provide this, but his receptionist told me that I should bring this as they do not have copies. They also want $50 to complete the form (which compared to other quotes of up to $500 seems reasonable).

However, I don't want to pay $50 to have a Dr transcribe information from one USCIS form (the DS-3025 that I already have) to the I-693A (which I am not even convinced is an official USCIS form since I cannot find it on their website).

I called USCIS who told me to read the I-485 instructions (which I had alredy done). When I read him the instructions, he told me that I would have to submit the Vaccination Supplement. When I asked for clarification on what form this was, he said that I should refer to the I-485. I have been VERY unimpressed with the level of help I have received from USCIS representatives to date, and dont actually think they know anything at all. I always seem to be telling THEM the process.

Anyway, any help would be greatly appreciated.

And if anyone knows the official location of the I-693A that would be great!

We just used the normal I-693, the vaccination information is on the 2nd page. There's no escape from the transcription thing that I know of...we paid $25.

The I-693A is outdated and does not exist.

Have a doc transcribe to the normal I-693, only filling in the vaccine information, or take a chance and just mail in the DS-3025 with your AOS.

You should not have to pay much for the transcription, but there are some docs out there who will try to overcharge. We paid $15, others paid $10-$50. More than that is unreasonable.

I am not even convinced the USCIS customer service know a single piece of information about immigration and what they do know they probably read directly off of the USCIS website.

I used Dr Arnold to transcribe my I-693 for $20 dollars, they tell you everything to send and certainly know what they are talking about. If you have any missing vaccinations (I had the silly HPV) you will have to get your I-693 transcribed.

Have a search for Dr Arnold on the forums, you mail him your information and they reply promptly with your sealed envelope and copies.

20/4/2009- Received NOA for AOS/EAD/AP (Dated April 17th)

20/4/2009- USCIS cashed check for $1010

09/5/2009- Biometrics

11/5/2009- Case transferred to CSC

05/6/2009- Received AP

09/6/2009- Received EAD

22/7/2009- Approved!

27/7/2009- Greencard in Hand

Removal of Conditions.

15/5/2011- Received @ VSC

23/5/2011-NOA1

12/07/2011- Biometrics

What we did was: we have the DS form, but we also followed the instructions on the I-693 Form, it states that K1 Visa you need to complete first part of I693 and then have civil surgeon fill in the vaccine supplment section and the civil surgeon will seal the envelope.

Thats what we did...

I think just to be on the safe side, its better to use fill out the I693 form.

$50 BUCK is alot easier than waiting for RFE ... and pro-longing the process *_*
