Timeline

My wife and I are collecting our documents to send off our AOS papers from a K-1 visa

We are a little confused about the vaccination report......

Do we have to get a doctor to copy the info from my DS-3025 to a I-693A or can we send the DS-3025 as a supplement?

Must get a USCIS civil surgeon to do it.

K1: 01/15/2009 (mailed I-129F) - 06/23/2009 (visa received)

AOS: 08/08/2009 (mailed I-485, I-765, & I-131) - 10/29/2009 (received GC)

If you had all your vaccinations in the UK just a copy of the DS-3025 will do. If you had any vaccinations since arrival in the US then you need the I-693A to be filled in by a civil surgeon :)

Yea, I forgot about that. In our case additional shots were administered in the U.S. so we had to go with a civil surgeon.

Where do we get one of them? And the doctor who did my DS-3025 was a USCIS approved civil surgeon!!

If you need one and want to get ripped off, choose one from the list of CSes closest to you on the USCIS list. Otherwise, do a search on this board for Dr. L E Arnold who will do it via mail-order for $30.

Thanks, that makes a lot of sense.

I haven't had any additional shots since I was here and I don't need any.

It does seen slightly insane to pay a doctor to transfer data from one form to another........

Great for you!! Many Vjers, including myself, have a hard time determining if the CDC immunization requirements were met.

for my case I took a risk of sending DS 3025 rather than my I-693A. For an additional vaccines are a lot of costs. anyways, me and my husband are crossing our fingers not to receive any RFE in the future until i get my GC. well, i heard other VJers we're doing that too. just that again we we're just taking risks.

Yeah, I get that feeling too. It just seems insane to pay a doctor to copy some writing from one piece of paper to another seeing as the first piece of paper came from a USCIS approved doctor!!

You need to include I-693 in your AOS application just to make sure. Some adjudicators consider DS-3025 but some don't and issue RFE.

I had my DS-3025 transcribed by a civil surgeon for $10. At first, they want me to have the full medical exam (but I already had a medical exam 7 months ago) so before I go to the clinic I brought the I-485 Instruction and read it to them. The nurse asked the cashier to charge me $10 for the paperwork and then took my DS-3025, waited for 10 minutes then she gave me a sealed envelope with a "do not open" sticker.

I include that sealed envelope to my AOS application and thank God I have no RFE and it was transferred to California Service Center which means no interview.

Best not to take the chance of just submitting the DS 3025. Dr. Arnold will fill out the I-693 for $30.

Why take a chance of having to do it later?

K1 denied, K3/K4, CR-1/CR-2, AOS, ROC, Adoption, US citizenship and dual citizenship

OP, please note that you if you decide not to send the DS3025 and get a civil surgeon to fill out a vaccine supplement, it is no longer the I693A. The I693A is no longer valid as of June 2008. You will need to use the vaccine portions of I693.

10/22/2007 - I129F APPROVED (161 days), I130 APPROVED (188 days)

11/08/2007 - I129F received at NVC, embassy case number generated.

11/13/2007 - I129F forwarded to embassy.

11/18/2007 - 129F petition received at embassy

01/09/2008 - finally, DOS gives me the interview date, April 16, 2007 (ouch)

01/23/2008 - never got packet 4, emailed embassy

04/11/2008 - picked up packet 4, did medical

04/14/2008 - medical report pickup, no problems

04/16/2008 - interview date- APPROVED!!!!!

04/18/2008 - both of us are home at last, POE JFK!

05/21/2008 - sent AOS and EAD

05/27/2008 - received NOA1 for AOS and for EAD

06/02/2008 - received Biometrics appt letter

06/19/2008 - Biometrics appointment scheduled - DONE

06/19/2008 - both AOS and EAD touched because of biometrics

07/29/2008 - EAD approved.

05/13/2009 - AOS Interview - APPROVED!/ Card production ordered email

05/18/2009 - Welcome Letter received

06/12/2009 - Second card production ordered email

06/19/2009 - Approval notice send email

06/22/2009 - Green Card received

04/09/2012 - Applied for Citizenship by Express Mailing N400 to NBC

04/10/2012 - N400 received by USCIS

06/23/2012 - Biometrics appointment

07/27/2012 - Appointment scheduled for N400 interview

09/05/2012 - Interview passed, oath ceremony completed, and Naturalization certificate received.
