middle name in green card (help!)

Hello VJ's members:

I already received my welcome note from USCIS, and my green card is production now. When I had the interview last Aug. 18, they said I must change my middle name, because when I file the I-485 I used my maiden name, which it is natural tradition from philippines. Now, they want me to use the middle name when i was a single. I'm so worried because all my papers now, driver's license, passport is already in my maiden name. They said I will petition to change name if I want to use my maiden name.. Of course I want to use that because they know me in that name.. What kind of form I'm going to use for changing name? I hope you can help me.. Thank you..

Here's the example in middle name problem:

Diana Santos Torres - when I was single

Diana Torres Smith - when I applied all documents

Diana Santos Smith - they put in my green card.

May 21, 2012 - Mailed N-400 at Phoenix, AZ

May 24, 2012 - Checked en cashed

May 25, 2012, received NOA letter with Notice date May 23, 2012

June 25, 2012 - received Biometric appointment letter, scheduled July 19, 2012

August 03, 2012 - received the initial letter

Sept 04, 2012 - Interview date

Nov. 02, 2012 - Oath Ceremony

What's on your marriage contract?

In our marriage certificate, it indicate my single name.
