I-693 and/or I-693a Required for AOS?

The latest set of forms I693 from USCIS requires the supplemental vaccination part to be filled out by a CS even if done in your home country.

The information on this site in the FAQ is WRONG, and has not been updated. The instructions on the new forms clearly states this. (06/05/08)

Now some people recently have gotten through without submitting it, however, my guess is it is just case workers who have yet to adopted the latest policies. When we had our interview, the gentlemen asked for the sealed I693 vaccinations from the CS. We did not send it in with the AOS, just gave it to them at the interview.

Actually the instructions to the I-485 say:

If you are a K-1 fiance(e) or a K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiance(e) visa, you only need to submit a vaccination supplement, not the entire medical report. You may include the vaccination supplement with your form I-485.

Nowhere does it identify that the vaccination supplement is only the Part II Item 5 of the I-693. Anectodal evidence would indicate the the DS-3025 also qualifies as a valid supplement.

The DS-3025 does not qualify as a valid supplemental for this unless it was completed by a CS, and even then it says in the instructions (Section III, Part 2, C, B) vaccination section must be completed by a designated CS.

DS-3025 and the I-693 are different forms prepared by different physicians/surgeons... A CS will not fill out a DS-3025 and a panel physician will not prepare an I-693. You are right that if you need a vaccination supplement prepared stateside you have but one option. An I-693 prepared by someone other than a CS is not valid, but that does not automatically rule out the DS-3025 as a valid supplement. It is hard to argue with the fact that the DS-3025 is routinely being accepted, so you can continue to argue whether it is a valid supplement or not, but until such time as it is not then the option is available for the applicant to decide what they want to do. Caveat Emptor. YMMV.

Not was... still IS... there are still many reports that people have posted recently (even as recently as within the last one to three months) of individuals who have been approved WITHOUT interview at CSC with having only submitted the DS-3025, despite the form change now having occured almost 18 months ago. Maybe the winds are a changing.... but until it is definite, then it still is an option for applicants to consider...... knowing the risks/rewards of how they put the package together.

I've contacted the much quoted Dr Arnold and they have given me instructions on what to do. Even informed me I require a titer ( as my records dont show I had chickenpox when i was young), so getting that together, sending off the info to them, and hopefully that we will be that!
