Any format for the AOS cover sheet?

Hi is there a format cover sheet to state what all documents are in the I864 package?

Dont want to get this wrong although it sounds simple.

Also what is teh correct and current mailing address to send the I 864 to???

Thanks Sherpa i searched this link and was not able to locate a sample cover letter for the AOS package....there were so many links which i kept checking.

Use this format, it worked for us:

: Copy of Petitioner’s last 3 tax returns and W-2s from the years 2008, 2007, 2006

: Copies of Petitioner’s last 6 months of pay stubs

All the XXXXX is where you fill in your pertinent information, okay? Organize the package according to this letter -- your bar-coded cover sheet goes on top of this. Make a full and complete photo copy of this package for you and your spouse.
