Family name is the Last Name of your father. Maidain name is the your Mothers last name before she married and changed her last name to your fathers(If she ever did).

This form is being submitted in connection with an application for Adjustment Of Status to a permanent Resident.

Most of the AOS forms are self explanatory, with the help of the filling instructions. You seem to be having a problem with the english and understanding.

thanks sapakbhai,

let me make my self clear. first of all i have to write my information on G-325A even if i am applying for my parents.

so out of 4 pages of G-325A i have write my information on 1 page and rest of 3 pages for my mom or dad. am i right?

also above questions is Family name is the Last Name of your father. Maidain name is the your Mothers last name before she married and changed her last name to your fathers(If she ever did).

so if i am writing my information i have to write family name, and in Maiden Name i have to write my mom's last name & in first name her first name?

2. this form is submitted in commection with an application for: Naturalization, satus as perment resident and other(specify) --in this if i am applying for my parents and i have to fill this form for myself what i have to select?

First of all, You have to submit a G-325a for yourself and each of your parents. That means you have to fill one set of four pages for yourself and 4 pages each of the G-325a for your parents. I read somewhere on this forum that there is a new form G-325a where you only have to submit one page instead of the older version where you have to submit four pages each.

When you are filling your G-325a you have to write your first, middle and last name. Fathers first and Last(Family) name and Mothers first and Family(Maidan Name being her last name before she was married).

This form is being submitted in connection with an application for: X status as permanent resident.

Just curious, do you know what the US Citizen is supposed to indicated in this box? We are filing the I-130 and I-485 concurrently and have checked the Status as Permanent Resident box for my foreign wife. Should I also check the Status as Permanent Resident box on my G-325A

Thanks!
