Proof of Medical Exam from K-1 Process

Hey there,

We're getting ready to apply for AOS, and my husband had his medical when we went through the K-1 process, what kind of proof does he need that it got done?

They never gave him any form or anything other than a vaccination checklist, the rest went into the brown envelope they took.

What you need to do is get form I-693a filled in by a designated civil surgeon.

This form is the vaccination supplement. For the civil surgeon to fill in the form they will need some kind of proof of the vacinations that have been had.

The form MUST be filled in by a designated civil surgeon. You can find ones in your area on the USCIS website here:

However, that shouldn't apply to us, he's already had the medical exam done during the K-1 process, therefore, it states you don't have to do it again.

However, what do we need to prove that it has been done.

You do not need to prove that the medical has been done and you do not need to have it done again but you DO need to submit the vaccination supplement (The medical is form I-693 and the vacciantion supplement is form I-693A). The brown envelope taken at entry will be with the immigration services already.

Yes this should also include vaccination information but this is the process. The requirements for this are clearly stated on on the form I-485.

I see that you are in or near boston. I can recomend the civil surgeon that i saw in somerville if that is close to you.

No problem. If it wasnt for these forums i wouldnt know any of this stuff myself. The Doctor I saw was:

He had an appontment the same day, had experience of the forms and had no problem filling it in from the information that I had. I seem to recall that he charged in the region of $60-$70. You may be able to get a better price if you shop around but make sure you see someone with experience of just submitting the vaccination supplement. Some will mistakenly tell you that you have to have the full medical and will charge several 100 dollars

It would appear from the experience of others here that if your vaccinations were complete during your K1 medical and you do not require any additional vaccinations, that you won't be asked for the vaccination supplement. The supplement is to update your record of vaccinations in the event that you had not satisfied them fully at the previous medical and had to have additional vaccinations afterwards.

Johanna got at the Doctor’s in Colombia a U.S Department of State Vaccination Documentation Worksheet DS-3025 and this is what we sent in with her AOS package.

Maybe. Or maybe not.

My wife did not, as the DS-3025 Vaccination Documentation Worksheet from her visa exam was considered to be adequate. Not everyone has this experience, but many do.

I have seen several cases like yours where the DS-3025 was deemed acceptable, i have also seen cases where it was not.

I guess it is down to the opionion of the OP. If they want to take the chance then that is their choice to do so. Personally I didnt want to risk getting an RFE based on the whim of an immigration officer wanting the I-693A as requested in the form instructions.

It is down to personal preference and what they think is the best course of action for them
