Howdy Everyone,

We have received our interview letter for the AOS.We knew that she would have to get the Vaccine Supplement But we were waiting to make sure with the letter.We have the DS-3025 from the origanal exam and she got all of the shots requiered for the visa and AOS in Thailand.Well we have now begun our journy to TRY and get the missing Vaccination Supplement to the I-693!!!!!This is the first time that we have had any problem in our journey.It Is CRAZY.All but 2 of the authorized Dr. in our area no longer work with the Government.I wonder why?One does not have the supplement.The other wants $75.00 US to file out the paper!!!!!Come to find out our primary Dr. does the whole medical exam and puts it in the sealed "brown"envelope.He is not on the posted USCIS athorized Dr.s list but according to him he has been doing it for years and is authorized.He has been my Dr. for many years and I belive him BUT.You get your shots from the local health department and bring the record back to him.He adds that to the exam work that he did and presto!!!! $225.00 US We will have her Vaccination Supplement and a medical exam that she does not need (she got it in Thailand from the US Embassy approved Panel Physician and all of the shots)He says that he does not belive that he has the authority to do the supplement by it self.I really do not want to give the 1 doc $75.00 US just to copy some check marks and sign his name but.........................it is lees then $225.00 US Any ideas???

07/12/04...Yai recieves letter with instrutions about interview from US Consulate with invterview date

24/02/05...Pay Visa fee in Bangkok at Post office

01/03/05...Yai has visa interview

02/03/05...We pick up Visa!!!!!!!!!!!!!!!!!!

02/06/05...Welcome to the U.S.A.

04/15/06...Offical marriage in U.S.A.(Tax Day) ;) ;)

Everything we have done was without the help of an attorney.We have not had any problems thus far and will not,I believe.

Timeline

I had to obtain the Vaccination Supplement for my daughter's upcoming AOS interview next week. All I did was fax in the copy of the Vaccination Worksheet that we were given at her medical exam for her K-2 Visa. The Civil Surgeon we found transferred the information from the worksheet to the Supplement and only charged us $35.00.

A new medical exam is not necessary as long as the K-1 Visa medical exam was less than one year prior to the submission of the AOS paperwork.

Maybe you could find a doctor that would allow you to fax in your wife's worksheet and mail you the completed supplement, for a lot less than $225.00.

If you update your timeline, I can be more helpful (I don't know if you got your AOS interview and when it is). Anyhow I didn't send a vaccination supplement either - just the DS 3025. I have my interview on Friday and I will post what happens.

Have you tried zip code lookups of areas outside your primary area? You might have to drive a bit but if you can get the price down it may be worth it. Good luck

Ya just gotta love the money grubbing guys who want to change the rules so they can charge you for something you don't need. Go to Technical Instructions for Civil Surgeons and print out the Technical Instructions to Civil Surgeons for Vaccination Requirements, you will find the information you need on page 1, but you might want to take it to your doctor to help him remember the requirements. :P

Thanks for the replys.The medical was Nov/2004 in Thailand,recived K-1 visa March 02/2005, married in USA April 2005, apply AOS May 2005, Receive AOS interview letter Feb 04/2006 for April 2006 interview.Everything is in order as far as what is requiered except the Vaccination Supplement that we seem to be haveing trouble with.

Be careful with making sure it is a recognised Civil Surgeon - check out the thread by Faith on AOS Denial due to an incorrect vaccination supplement and supposedly the Doctor not being on the approved list!

You gotta remember, Doctors know it all and telling him is not always enough, showing him the CDC requirements puts him on notice of the rules and charging you for an exam you are not required to have falls into the fraud category and I am sure he is smart enough not to play that game.

We paid $70 just for the supplement. It was excessive, but saved us time trying to shop around for the best price. I would just pay the $75 and be done with it!

It is not so much the money as it is they do not deserve it.

Do you need the vaccination supplement to submit with the I485 or only for the interview?

We submited the DS-3025 which is the vaccination documentation worksheet from the origanal medical exam in Thailand with the I-485,we had a copy.But all of the paper work for the interview asks for a Vaccination Supplement to the I-693 filled out by a civel surgen for the interview.

The I-485 instructions say you "may" file the supplement with the application. But I would send it in with the application though.

We wern't even asked for the supplement at all. I never even got the supplement done.

I had given Immigration a copy of my Vacination records from my K-1 Medical but that was all.

That was one thing that worried me tho. I thought that they would ask for the supplement and since I didn't have it, would be denied. But it was fine and I'll be getting my greencard in 2 weeks.
