I have read the guides and searched and read posts here about the I-693 but I'm still unclear ...

My fiancee had her medical exam in June, we will me married and file for AOS next month. I have her DS-3025 (Vaccination Documentation Worksheet). She needed and recieved a vaccination followup here and we have her vaccination record from the County Health Dept.

Do we need to complete a I-693 or need the services of a civil surgeon before we file for AOS?

Thanks for any replies.

From what i can gather from reading the forums, most people who had all their vaccinations at their medical exam prior to moving to the US, have just used their DS-3025 (this is what i did) and no one seems to have run into any problems. If vaccinations were recieved AFTER the move then the I-693 was filled in by a civil surgeon.

I hope this helps.

Good luck

Yes, she needed a follow-up Tdap 4 weeks after her initial one. So I need to find a civil surgeon to just complete the vaccination section of the I-693? Who is this Dr Arnold I keep reading about? He can do this for me even though I'm in Michigan and he's in Texas (I believe?).

Shots have to be signed off by a US government approved doctor. So if all shots needed for AOS are signed off at the overseas medical on the DS3025, then it's enough.

But if you get more shots in the US, then an approved doctor (civil surgeon) is the only one who can sign off. The official form he uses is I-693.

Shots after arriving in US = Must have Civil Surgeon and I-693
